I think you’re pushing sx here. I am a 61 year old retired Marine (jumping/rappelling out of helos) and a truck driver (3-12 hour shifts per week, loading and unloading 3 to 600 pound surgical cases every Friday, Saturday, and Sunday. I also am 40lbs overweight, have triglycerides levels over 500, HBP, and Morton’s Neuroma in both feet. I also drink too much on my days off and am nowhere near the 180lb “fighting machine” I was coming off Parris Island boot camp in 1986 (33 years ago) I think I will try a few things before I let someone cut my nerve! 🫡

Good morning Sir, My name is Dr. Jay Shah, an Orthopedic Hand Surgeon from Mumbai, India. I’ve been experiencing chronic left inguino-scrotal pain for the past 7 months. Despite my family's strong medical background-my father and grandfather are both MDs in medicine, and my eldest brother is a Hepato-Biliary Oncosurgeon in Mumbai-this case appears to be quite rare. None of the doctors we've consulted have come across something similar. As someone who has followed your work closely, especially due to my interest in hand surgery, I’ve noticed that you’ve managed several cases with similar presentations. I believe your expertise could shed light on the best approach for my treatment. Recent MRI neurography reveals the following: - Asymmetric thickening and increased signal of the left ilioinguinal and iliohypogastric nerves, as they course between the internal oblique and transversus abdominis muscles in the lower anterior abdominal wall and into the inguinal region, with the ilioinguinal nerve entering the spermatic cord. - Mild asymmetric thickening and increased signal of the left spermatic cord. - Maintained contiguity of the nerve fibers, without any nodular thickening, mass, or cyst seen in the canal of Nuck. - No overt perineural fibrosis is observed. - Subtle thickening and increased signal of the left inguinal ligament. - Surrounding muscles and adjacent vascular structures appear unremarkable. I’ve been in significant pain, which is now impacting my daily activities. Unfortunately, oral prednisone and pregabalin have not provided any notable relief. Could you kindly advise on the best course of treatment moving forward? I look forward to your response. Thank you. Sincere regards, Dr. Jay Shah

I'm experiencing nerve pain from left inguinal hernia surgery right now. It's brutal. A car ride feels like knives plunging into my testicles. Luckily, I have great health coverage and rather than going in circles I know exactly what it is. I will request surgery with a nerve pain specialist.

Hi Doc. I burned my feet in hot water/mag surfate salt bath. I have pain and burning mostly when I’m walking or standing. Although this was a first degree pain, I’m wondering what has happened and how can I reverse it.

I injured my back lifting heavy tiles improperly 11 months ago. The pain is on my right side, radiating to my right hip, lateral and anterior right thigh. The MRI shows disc buldges and disc protrution at L2, L3, L4, L5, facet hypertrophy and ligamentum hypertrophy throughout lumbar. I get relief from laying in bed and with Mckenzie extentions sometimes, but sitting and standing too long at work brings it all back. My symptoms seem like meralgia parastetica with low back pain. What can I do? It is destroying my life.

Why won’t you take insurance? Other doctors performing this operation take insurance and the patient pays the difference . Why can’t you do this so people can get help? Whatever insurance pays you accept but patient agrees to pay the rest. Win win. All you have to do is opt in with insurance and explain this to your patient .

No, it's not. My nerves are 50% dead according to a biopsy. You're talking about entrapment which would only affect one leg or one arm at a time.

Mine is constant and can be debilitating, hard to walk, work and depressing. I had a recent radical robotic Laparoscopic nephrectomy and for 2 weeks the pain was gone just numbness in upper groin and right leg. Now pain is back and though I'm in still recovery I am finding it very depressing and painful.

@@varunsharma6217 hi thanks for the comment. I am recovering slowly from nephrectomy surgery. Still sore in places with movement but being careful before going back to work as it's very physical. With regards to the groin pain, that is still here. I've had it before the nephrectomy so I don't think related to cancer. But I went over ultrasound result and except for a bilateral epididymis cysts & atrophy of right testicle there is no abnormality. The pain is only in right side. I have been looking through medical history. When I was around 2 I did have surgery for a undescended right testicle. But I don't know why I'm getting these pains can't be "blue ball" I get this pain even with no excitement. I can get pains from walking or even just standing for long periods. I'm seeing urologist surgeon in 2 weeks so I'm going to consult with him. I can't live on Advil for rest of my days.

I started zinc 14 mg a day by day 5 my foot tingling, numbness in toes and pain pressure point slowly began to subside currently at 10 days symptoms have decreased from a level 5 to a 2. Can’t say for sure it was the zinc but that has been the only added supplement or change to my diet and intake in the last month.

I can’t believe you have it so “right on”. You are speaking right to my situation throughout the peripheral neuropathy video. I sure wish I had watched it live. Better yet I wish I could have a discussion with you concerning my situation. Just a little history, no I changed my mind. I forgot this is viewed by the public. I have diabetes, diagnosed first as type two, then rediagnosed years later as type one. The neuropathy came later. I’m sorry this is too much information for you to have to read and for the public to see. How can I get in touch with you? I have seen endocrinologist, neurologist ,cardiologist almost every kind of ‘olgist ‘ practicing. And no one is getting it right. Meanwhile, the condition is probably worsening and I am very very uncomfortable. I can see & feel the situation worsening. It’s almost like they don’t hear me. But you have heard somebody because you’ve pointed it out throughout this lecture. I could give you hundreds of reasons why I should not give up. However It seems hopeless since Im not diagnosed correctly consequently not treated. I know this because I’m not getting the evaluation or a recognition and/or explanation of my clearly described symptoms. After listening to you, I’m certain . I need some help. I live in Hampton Virginia. I will come to you if possible unless you have another idea,a colleague perhaps but not preferred. I know one thing : we can’t diagnose, treat, follow and evaluate through a podcast. I’m stuck please unstick me. I can I think describe to you everything that is manifesting as signs & symptoms. . You ,sir are brilliant. And I believe you can help me. But I salivate over maybe a bone you might throw. Wow it’s 4 am . Thanks for reading this if you are ,if you’re not and someone is, please pass this along. I’m serious about needing some help. I’m stuck here. My email is RRILEY [email protected] THANKS , rr P.s. I have a tremendous amount of pain in my right, but it feels like it’s in the bottom of the bone. I can’t sit on it for any length of time. That’s not all but that’s a big one. I can’t sit for more than one or two minutes. Also, my son and I are coming to Las Vegas in September to see Shania Twain, but I’ll be able to see you then. Be nice to be able to talk.

I am a 68 yr old female, thought I had Sarcopenia until I found a video on this subject. Now I know exactly what the problem is, I can treat it! Seen numerous doctors/physios, about my hip and leg pain, not one of them has suggested that. I may have Meralgia Parasthetica, just keep given me painkillers.

I can't find any literature on Piriformis Syndrone caused by Bulging Disc. The Bulging Disc is not healing. I've tried the chiropractor, the injections and the Physical Therapy. I can't have surgery on the Piriformis if the cause is the bulging disc. My doctor doesn't recommend surgery, but the pain doesn't go away. The more I stretch or exercise, the worst it gets.
