When I flew for an air freight company, we had to make sure the ground crew placed those sideways, so if one blew, it would go out the side of the airplane rather than through the cockpit.

Years ago a Chicago-area steel mill on the lake shore was losing oxygen cylinders at a large rate. Security increased patrols and inspections. Still losing them. Bosses were doing random checks on vehicle in the mill. Nothing. Then a complaint was received from a ship passing the mill, that it was almost hit by something launched from the shoreline. It was discovered that a couple of guys were knocking the valves off the tanks with a sledgehammer, and launching oxygen cylinders using an old I-beam like a launch rail. They admitted to launching a few dozen tanks like that. They were fired.

We had a garage set aside for outside contractors at the tire factory I worked in. While not my job, once a month or so why going to a nearby area I would go through there and point out tanks that were not chained in place, held in with bungee cords and such. If there was someone around I pointed it out. If it was taken care of, no problem, if there was no one around or they didn't fix it I would notify plant safety. In my younger days I worked in a tool and die shop that had a hole in a concrete block wall from a tank.
