I tell my patients that it's not possible to reverse, but one can slow the progression of CKD by:

1. Controlling blood pressure ideally to < 130/80, 2. Lowering urinary protein (controlling BP with RAS blockers also helps with this). 3. Avoid medications that are toxic to the kidneys and 4. Be healthy (exercise/ diet) whatever is good for the blood vessels will be good for the kidney.

can low anion gap be caused by keto and dehydration? i have 3 and it is indicating as low. I do eat a lot of fat. bun/creatinine ratio is elevated. I take magnesium supplements (magnesium was not measured) . everything else is normal

Look at dry fasting for kidney benefits. A small trial of 5 days showed an inprovement in renal function of over 160%

It depends, things to ask your doctor are is the creatinine stable, is there proteinuria or an abnormal urinalysis, is there an abnormal renal ultrasound or is the blood pressure elevated

Thank you for this information. How about age 44 with a 1.2 creatinine level with a 59 eGFR for stage 3a? No diabetes nor hypertension and does exercise frequently and eats relatively a good diet.

Things to consider and ask your doctor are if the creatinine and eGFR are stable and if there are other abnormalities on urinalysis, urine albumin or ultrasound

@ thank you. The eGFR has been at 64 as the best and 57 as the worst in the last two years. This is with 6 different examinations. There is no abnormalities in the imaging. Creatinine levels range 1.1 to 1.2.

My egfr 40 last 35 day ago. Now increase to 59. I just controlled my protein intake, cut sugar and monitor salt intake. Everyday im skip my breakfast. Hope my egfr back to normal within 2 months

Yes exxxaacctly! Mine went from egfr 46 to 62 in 1 month. All I did was quit sugar and carbs and drink more water. I didn't even realize how addicted I was to sugar and coffee. I think next time my egfr will be even higher and I didn't cut down on protein. Truthfully i increased protein by moving to a keto diet.

@@BCNephro Yes I think those 2 are the key. That was quite a scare for me because the doctor said after 3 months I would be diagnosed with stage 3 kidney disease. I had no idea what I was doing wrong at the time, or that there was a problem.

Why would the development of Calcium oxalate crystals due to ethylene glycol toxicity damage the kidneys since they only form kidney stones and these can be easily removed.

I've never checked a calcitonin level. This blood test is used for the evaluation for medullary thyroid carcinoma, not for the evaluation of hypercalcemia.

@@BCNephro Do you always see D 1,25 low in subclinical hypercalcemia where blood calcium isn't yet out of range? I've been talking to a few people that have falling eGFR and low PTH but claim to have normal D levels.

A low 1,25 vitamin D level would be expected in CKD with secondary hyperparathyroidism, with the elevated PTH maintaining the calcium in the normal range. However I don't typically check 1,25 vitamin D in this situation. An elevated 1,25 vitamin D can be a cause of hypercalcemia in granulomatous diseases like sarcoidosis or lymphoma. This would be a part of my evaluation in a patient with hypercalcemia and a suppressed PTH.

Yes guidelines recommend it for this indication. Although I typical reserve it for patients with overt albuminuria already on a renin angiotensin system blocker.
