Hi Doctor, Glad to see this informative video. I have a doubt regarding the tattoo removal 5cm×1.5 cm under collar bone. Black coloured. I am an Asian (India) with fair skin. I want to remove my tattoo amd I am confused whether to get it removed via laser or plastic surgery. Plastic surgery will also leave a scar and laser also leaves fading. Can any laser treatment make the skin looks tge same as before getting the tattoo?

This would really dependent on your skin type and skin sensitivity. Everyone's skin is different so it is good to closely monitor the condition of your skin while gradually increasing the frequency of use complementing it with moisturisers rich in ceramides or hyaluronic acid to keep your skin barrier protected. If you see or feel any irritation, make sure to stop using it immediately and visit a dermatologist to seek personalised advice.

I really want to try this out for crystal clear skin, even tone. And I want tighten my sagging smile lines. My face doesn't have major skin issues. Just slight discolouration on the forehead and jawline. My eyes and cheeks are fairer. I want a V shape face. I notice suddenly my face has become soooo round. I reserve all treatments for next year after the recent treatments done. Now, face is okay. I don't want to damage it with too many treatments.

I’m wondering what your experience with ultherapy has been with your patient population? I’ve read many anecdotal information from people and how it caused a complete fat loss of their face months after treatment. To me, it seems like an energy device that should not be utilised for the face, similar to radio frequency energy devices incorporating micro needling which also causes permanent damage to fat cells. However, I’ve read studies that IPL in the range of 500-560nm stimulates collagen production. In your expertise, which energy wavelengths and fluences have you found stimulate collagen production without damage to fat cells for energy devices such as pico and IPL?

Personally, my take on it is that the effectiveness of energy-based devices depends on selecting the right treatment for the patient, understanding skin and fat thickness, and using the correct transducer depth and shot count. When performed by a well-trained professional, risks are minimised. However, improper use by less trained practitioners, such as applying the wrong energy levels or depths, can lead to issues. Devices targeting superficial layers tend to have lower risk of affecting facial fat deposits. Hope this clarifies!

I’ve read in a recent South Korean research paper, that male pattern hair loss is correlated to diabetic conditions and that transdermal injection of pentapeptide 5 attentuates both the hair loss and the diabetic condition. As far as female hair loss, I’ve read it has to do with under functioning thyroid.

This is quite fascinating. Dr. Yong, do you find polynucleotides more effective than dermal fillers with poly-l-lactic acid such as sculptura? I’m curious what your thoughts are and what you’ve experienced in your patient population. IMO I think skin boosters like HA often make people look puffy and pillowy, which actually makes them look older and not a desirable look. However, I have seen higher molecular weight HA (profhilo) utilised on women’s underarms with crepey skin to completely eradicate the crepey skin to make it look firm and toned. I’m curious about which procedures you have done on yourself (if any) and where you tend to inject polynucleotides on your patients? I do know that in South Korea, rejuran (salmon sperm- a type of polynucleotide) is injected during micro needling sessions. However, I do not think I personally want salmon sperm injected into my skin. BTW I love how you explain everything quite thoroughly and try to educate your patients, I always think that is the sign of an excellent doctor. 😊

Hello @ngoctrangle-alfonso9771, in general for skincare, antioxidants are good for most/all skin types, hyaluronic acid in a serum or moisturiser, niacinamide or other soothing calming agents like heartleaf can all be useful. It is recommended to incorporate a good antioxidant serum including a good potency vitamin C, broad spectrum sunscreen with at least Spf 40-50 and PA +++ in the day, and depending on skin type, using a retinol or retinoid 2-3 times a week can be helpful to preventing fine lines and photo-ageing. For injectables, some good products that improve skin quality, consider skin boosters that contain hyaluronic acid to hydrate the skin, polynucleotides to repair the skin and improve fine lines/pores, and also certain bio-remodelling injectables can be helpful to improve elasticity and hydration safely. At the end of the day, skincare and treatments are not a one size fits all and will vary depending on your skin's condition and your history. Do make sure to seek a professional's help for customised skincare to your skin type and condition if you have trouble addressing it yourself!

I've been using a salicylic acid cleanser, cleansing oil, glow serum, and snail mucin essence for 2 days. My skin is oily and I was double cleansing twice a day. I noticed more pimples, including red ones, so I stopped using everything. Help! What should I do now? How often should I double cleanse?

Do not double cleanse two times ,use Salicylic acid cleanser only 3 times in a week,snail mucin might not suitable for everyone so stop using essence and use a gentle face wash , moisturizer and a sunscreen and double cleanse only if you use makeup or water resistant sunscreen you don't need too much cleansing it will strip your skin barrier

Cleansing and a good cleansing routine whilst essential and helpful at managing acne prone skin, should also not be overdone, and double cleansing can be done just once a day instead of twice. Generally, we don't want to strip the skin of too much oils and moisture which can then lead to further breakouts as the oil glands might try to compensate and produce more oil. This may actually be more counter productive and lead to more acne formation. Hence, it is advisable to only double cleanse once at night - using an oil based cleanser/cleansing oil as the first step and then a regular wash after that which can be one with salicylic acid. It would be good to be cautious with salicylic acid cleansers depending on strength and sometimes it is recommended to use that in rotation, for example using it only once at night and using a more gentle cleanser in the day without any AHA or BHA so as not to overdry the skin.

Hello! HUGE FAN of your informative videos! You are the laser guru! Question: Is it okay to get 3 different laser treatments at the same time (mainly forfacial redness + a few small broken vessels around my nose, but also for some PIH from old acne)? I'm going to Asia and hoping to combine Picosure (755nm nm), Cynergy MPX (595 nm), and a vascular laser (Wontech V-Laser) that uses 532 nm within 1 week. Is this overkill? For context I'm Chinese with fair skin, dark hair, overall very good skin but always with a flushed appearance on my cheeks and nose. THANK YOU!!!

Hello @sweeetsinx, thank you for your comment! Generally, yes it is ok to get 3 different laser treatments even on the same day or the same week as mentioned. It is quite common at AYD to treat patients with certain combination of lasers on the same day as lasers are very selective in what they treat, unlike broad band light, lasers are basically of a single particular wavelength, hence the laser deployed has a particular selective chromophore or target in the skin and this typically limits any collateral damage if deployed correctly with the right fluences (energy settings) for the right indications and adjusted for the skin phototype. However, the value of adding another 532nm laser as you mentioned for vessels is debatable. Most of the facial redness and broken capililaries can be treated with a 585 or 595nm laser such as the cynergy, and the pigmentation mentioned with the 755nm alexandrite laser, hence the 3rd laser may not be necessary in addition or may be duplicative to treat either vessels or even epidermal pigmentation with 532nm laser can be used to treat. Hope this helps! But treatment differs for all skin depending on the skin type and condition so please seek a dermatologist consultation for a more customised treatment plan.
