Have worked around that crane before it was an awesome piece of machinery! But at the end of the day it is and always will be the operators responsibility to make a safe lift!

You gotta wonder how much did this cost: the lawsuits, the damage to the existing structure, a destroyed crane, and re-doing the part that they were lifting originally. 20 times as much? A hundred times as much? More?

The sad part is somebody's got to get killed before the rules change and all she is always late to get there as usual and it always comes down the greed Zeus ever building the stuff they wanted done quick so they can make more money

4 separate operators didn’t make that decision. The original crew didn’t want to make the lift and quit. they Cowboy’d it and decided they were gunna go for it.

Safety is always top priority... unless you are behind, then the true "safety" of a company shines through. Wind gusts almost x3 times the limit for the crane and load, and they still attempted it. They gambled, and lost.

Why isn't the site foreman or project manager held accountable? End of the day what they say on a site goes,so they should of said to postpone the lift etc

That was so big!

There are some problems with this report. 1. They were ironworkers, not "steel workers". 2. The fault of 97% went to Mitsubishi, NOT the operators. While there was clearly some culpability on the operators part, there were systemic failures throughout the process including deletion of important wind speed data. It is troubling seeing someone taking the time and effort in putting a video like this together with misdirection and spin.

I think the regular operator refused to do the lift because of the wind.I think he went home,then they got someone else to do the lift if I'm not mistaken

Nothing new. Constantly firms seek the lowest bid, and to win it, the bidders will often unrealistically set expectations of their teams. We're in the contract product development business, and we see it all the time. The sales side of the bidders are of the mentality, "better to ask for forgiveness than permission". Thing is, it costs multiple times more than the original bid, is off schedule, and results in lost revenue not only for the time it's delayed but also to correct the issues created.

every crane that I have run had a wind speed in the cab. I bet there was one in that cab as well. the operators obviously knew what the wind was doing, you can feel it as an operator. being pushed by a boss to make an unsafe pick is just wrong! Doing an unsafe pick is on the operator. and we see how it can turn out. tragic. why were the iron workers in the basket and in the air? they wanted to get it connected quickly knowing the wind was an issue. on a site, everyone knows when things are dangerous.....just no one wants to be "that guy" that speaks up. sad but real outcome for not speaking up.

There is no such thing as an "accident" on a construction site. Someone always gets in a hurry, keep a schedule that is unrealistic or is afraid they will get laid off if they don't do what is asked. As a result, workers get killed. MURDERED is more like it. Those supervisors, with the "AUTHORITY" to say "NO", should have been charged with Negligent Homicide X3 AND put in prison breaking rocks. Yet, on other job sites...the beat goes on.
