Oh my goodness. We are so sorry to hear that this is your experience. Our clinical team members get paid extremely well! We can do so because they are so incredibly productive. But they are productive because we are so organized. The organization is what makes productivity possible. That, in turn, makes it possible to make sure that everyone gets a great paycheck! Maybe you need to look for a new office to work with?

What would you do if you have drawers on both sides of your room and there is no space for mobile units? I desperately want to get rid of the drawers on the doctor side since there is no wiggle room. I feel like I am always bumping into something

Your realization is beyond what most of us dentists can admit - We bought them. Probably paid too much for them. And they actually get in the way of doing more dentistry! I feel your pain. _Sometimes_ you can shift the patient chair off the midline in order to get enough room (I know that us dentists love symmetry and so this bothers us!). Unfortunately, the reality is that those side cabinets were designed for those 2 room offices in the 1950's when nothing ever left the room (remember, back then things were "sterilized" in a glass dish full of toxic goo). There was no technology to mobilize...because...there was no technology!

The problem for assistants is that we can teach them this - and they love the comfort, ease, and performance that they can achieve - but it becomes a tremendous frustration if their doctor and office are unwilling to change and improve how they work together! Oftentimes assistants that come to understand this have to find another office!

I suspect that what you are having a hard time believing is that an upright posture, like what you are seeing, actually allows visualization. This video is not staged. It is simply a normal day in practice for those of us that work in this system. I assure you that the assistant has perfect visual access to the entire procedure! Pretty exciting isn't it?

This probably sounds weird, but teeth are actually elective! We consider them vital. They are. But people choose to keep them. They pay us to keep them. They are therefore - customers! Not hostages. They are free to choose us ... or not, and therefore is our responsibility to exceed their expectations and deserve their loyalty.

Have you designed clinics in rural areas where there are ongoing shortages of dentist's? What's your take on such an issue? If you expand based on projected pph, but there is no dentist keen to work at your practice, is the expansion investment worth it?

Labor short areas are actually the location where this is most helpful! These are the areas where demand for services are so high! 2 important results occur: 1. You can double or triple your personal production - making the associate less imperative. 2. There are many doctors that will want to join you when they see what their income potential will be!

It can be more understandable when one sees the dentist as a very small (and amateur) business owner. Scarred from years of expenses that may or may not have paid off. Seeing the pain of these expenses - and unable to decide exactly what to do next or who to believe. Control freaks have a hard time letting go!

I’m a dentist at a FQHC. Never seen a dish washer in a clinic before. What things do you dish wash? Trays? Alginate bowls? Just wondering since they are planning to create a new clinic at some point

The method many dentists are used to is slow, with lots of hand labor, and the possibility for lost and misplaced instruments - and instrument sticks. Dr. Ahearn proposed utilizing dishwashers to remove bioburden prior to autoclaving back in 1993. Today it is our standard recommendation, although of course you should check with your local regulatory authority. How many squarefeet do you recommend for a hygiene room and for a universal room?

Dimensions of the room are more important than the actual square feet of the room. Hygiene Room - Typically 7ft wide by 11ft deep Universal Room - Typically 8ft4in wide by 11ft deep. At these dimensions the rooms become the most efficient, the dimensions are flexible within an inch or two. With that being said, we wouldn’t want to go wider than 9ft wide for a universal room because if you do, then you can’t reach the side walls easily (and we know you can’t use what you can’t reach). We also wouldn’t want to go any narrower than 6ft6in wide in a Hygiene room because if you do, it becomes too cramped and uncomfortable for the Doctor, Hygienist, and patient. Hope this helps! 😊

What about room for the anaesthesia ventilator, IV pole, emergency cart, oxygen, gas, and patient monitors? Those should go on the side of the dental chair to prevent the anaesthetist from contaminating the sterile dental materials and instruments behind the head of the chair. The anaesthesia circuit should be long enough so when the patient is intubated in the dental chair, the connection is directed away from the sterile field of the dental procedure. When a patient is under in the dental office, all IV lines, monitor cables, temperature support, and respiratory circuits should be away from the sterile operative site so the dentist can be sterile in his/her work while the anaesthetist manages life support.

Big smile here. We agree 100%. Plenty of room for that. The problem gets created when the rooms are overcrowded with supplies for procedures that won't even be contemplated for weeks! Generally in side cabinets.
