She is a Doctor that runs the Pokemon center while she specializes in Pokemon she has been shown helping people as well. All of the pokemon that work with her Chansey, Blissy, Wigglytuff, Audino, Comfey, Indeedee (male and female),and Jinkx who are are nurses or head nurses of the pokemon center. While other Pokemon that nurse joy may have are assistant: Sunflora, Bulbsour, Banette, Drifloon, Combusken, Maganium, latias, etc. they all help around the Pokemon center but not main nurses like the others

The owner of the pokemon centers are cheap and are making her 2 jobs at once. Like the office at a plumbing company i know of they fire people then the fires person's work gets pushed onto everyone else

So due to old stigma of females still being lesser beings even in the 90's it was likely changed to be nurse because a doctor was viewed as a male job and Nurse was a female job. This was likely an easy way to for more people to accept due to the stereotype of the time.

Get a Machamp, become a construction worker. Get a Poliwrath, become a firefighter. Get a Raichu, be a powerplant worker.

UPDATE: Actually, let's make this a comment for adding corrections. - I have since been informed by MercuryFalcon that The Poseidon Adventure came out in 1972, not 1983. I apologize for the misinformation and the responsible parties have been summarily executed. - Petsuchan on twitter politely told me that Sasuga no Sarutobi was not done by Ashi Pro, but rather Tsuchida Productions. I got a lot of different things intertwined in my head, but a little fact checking could've prevented that embarrassing blunder. - Petsuchan also suggested that two other characters from Sasuga no Sarutobi formed the "bridge" between the Gaikkotsu gang and Team Rocket -- Agents 00893 (Hakku) and 00498 (Shiku). They are a comedic duo and do have a more flamboyant appearance, so there's definitely some shared aspects there, but right now I think he really wanted to emphasize something other than just the Time Bokan-qualities that they definitely have. Still, Petsuchan knows far more than me and I take the connections he makes seriously.

Something I really think helped Shudo to get a chance to explore his ideas was that originally Pokémon had a foundation that allowed for such exploration. As much as the novelization existed to explore more Shudo's original ideas, they were made building upon Game Freak's original documentation for the Pokémon world. Many times in his blog posts he mentioned about Game Freak's original lore and how he was still building upon that, and I agree with him. It does get a bit ironic how many Pokémon fans will often think that much of the content from the early anime or other Shudo-related material are odd and "they don't feel like Pokémon", when it's just a case that the Pokémon most people know nowadays and the one Shudo worked on were two completely different beasts. The way i see it the Third Generation really was the turning point when Pokémon made the change from the more "grounded Alternate History world with new mysterious creatures" into a completely fantastical setting (In fact part of the original conception for the 3rd generation really was more like a reboot featuring a completely new world unrelated with the first 2 generations, which maybe could have helped to keep stuff separated to let the original concepts in Gens 1 and 2 still shine). If stuff was kept during 2nd Generation, I genuinely think Shudo could have done the ending he wanted, but the desire to let Pokémon grow much more really change things. Still, it's undeniable the influence Shudo had on Pokémon even in the more small parts of world building. In the manga Pokémon Special/Adventures for example, the inner workings of the Pokeball as having a capture net that could extend to really be what captures Pokémon was an inherited setting from Shudo's novel. Seeing how all these different minds developed the Pokémon lore in the old days is quite refreshing to see.
