I thought calcium inhibited votage gated sodium channels? Isn't that why you get tetany in hypocalcemia, i.e due to disinhibition of sodium channels leading to higher excitability of neurons. Are you saying it has the opposite effect in cardiac myocytes?

I am 47 and on the lowest dose of Ozempic, I always try to hit my macros and calories even if I am not hungry. This little bit of help really made me realize I LOVE strength training! I am losing at a slow but steady pace. However since taking it my psoriasis arthritis pain is 100% gone and my inflammation is normal! I have tried most of the biologics over the last 7 years! My Tremfya is so much more expensive than the GLP 1s. So this peptide has been a big win for me but it is only work it if you change to having a healthy life. Thanks for the video it is a great watch!

I'm on here listening to this because I just finished my FIVE day course of Cefdinir for a bladder infection. I have had dysuria and bladder inflammation at day 4. I've never even heard of this antibiotic in 8 years of getting UTIs.

I did not, I ended up going to urgent care and they gave me something else.

it was only a five day course and I felt a lingering discomfort for a long while until I got my next UTI. They did do a culture but doctor didn’t go over it with me. It says “non-uropathogenic gram positive organism.” No susceptibility was performed. I usually take bactrim or cephalexin but it has to be 7-10 day course.

This was very informative…thank you. I freaked out when a doctor prescribed Cefdinir for a complicated UTI (after finishing a course of nitrofurantoin-that should never have been prescribed in the first place). I was about to call in a favor to another doctor I know and ask for Cipro (I know, not great…but my kidneys hurt and I live in a small, remote town), thinking the Cefdinir doctor must be an absolute idiot for prescribing it to me for a complicated UTI. Now, I’m realizing he’s either a well read genius (nitrofurantoin did help quite a bit in the lower bits, and even somewhat in my kidney…though symptoms returned on the last day of the course), or an idiot…but, I’m willing to wait/suffer a day or 2 more before making that judgment call now haha.

This was a great chat, especially as I’m gearing up to provide a small education piece to my IM residency later this week at a noon conference. @@ReapingTheHarvest No, but lately it's been more difficult. I've been wanting to smoke the last couple weeks but I've stayed away from it. I have dreams every now and again that I'm smoking and I actually feel guilty! I guess being able to stay off this long it's become more of a goal to not smoke. I do see a difference in my sharpness at work. It's beginning to come back to me; I'm quicker and smarter solving problems faster. I've also stopped coughing so much and lost some weight. But damn, I'd love to smoke and I don't think that will every go away. I also want to say I think I was able to quit easier because I take methylphenidate for adhd. I can't exactly remember but somewhere I read methylphenidate acts on the system like weed. I totally believe this because I tried to stop so many times and failed, but since I've been on methyl (14 months now), staying clean has been 10 times easier.

Great talk. Agree, no clear dosing strategy has shown superior. For me: high risk pt -> 10 mg/kg -> triggered dosing with Pb (65 or 130 or 260) q30 min if going to ICU or bzd if going to floors so I don't confuse our house staff. Concur, HARD pass for people with drug interactions. Also care to be taken in hepatic dysfunction (personally, I dose reduce). High risk = prior seizure, intubation, ICU admission, looks terrible, prior ativan/versed gtt for AWS Would like to hear more about timing and interpretation of drug levels.

FYI regarding musher dogs... interesting that he discussed them and the Iditarod as it is a long-standing Alaskan tradition commemorating the dog sleds that brought diphtheria treatment up to Nome when they had an outbreak. Truly rich history of a community coming together for the greater good.
