If this is for your personal use, then why not shorten the goal description by 70%. Since this isn't a legal document, it can be colloquial. "BEHAVIOR: Sean will state problem, ID size, generalize other, offer solution." Boom. Done.

Hi, if I make a copy for each student and attach it to google classroom I will be able to see the results as they fill it out yes? Also if I want to send it to my grade level to use with their students do I still make a copy and send to them? and they will make a copy so that they only get their student results?
