I think you might be wrong in scenario 4. It's a mixed alkalosis as both co2 and hco3 are causing alkalosis. For a compensation to occur one of these elements needs to move towards acidosis.

The institute generating the original document should be sending it directly to CAPR. If its done separately, should be sent separately
