Differential diagnosis from history:_ SLE with CNS involvement ??? PANDAS kindly answer mam, can PANDAS be one of the differential
@learningpediatrics534715 сағат бұрын
Rheumatic Chorea often presents with carditis, whereas PANDAS does not. Given the presence of carditis in our patient, it strongly supports the diagnosis of Rheumatic Chorea with carditis

How cyanosis zero in downes score, baby is on oxygen by hood. Atleast 60-80% Fio2 is being delivered if flow is 6-8 L/ min of oxygen as per recommendation. .. So score will be 2 , Downes score is 8 for the baby, needs intubation..
@learningpediatrics53479 күн бұрын
this newborn presented without visible cyanosis at the time of admission. Despite requiring high oxygen therapy, some newborns may not exhibit cyanosis at admission. Certain underlying conditions can affect oxygenation and mask cyanosis, including: - Some congenital heart disease - Persistent pulmonary hypertension - Anemia or polycythemia These conditions can impact oxygenation and make it challenging to diagnose cyanosis.

I wonder how it feels to be one of those babies, probably like being set on fire and rolling around in rusty nails and bits of broken glass. But I know with age their skin does become tougher and many adults who look in much better shape who seem to enjoy life.

We can't detect it through USG?
@learningpediatrics5347Ай бұрын
Harlequin ichthyosis is a rare and severe genetic disorder. Antenatal ultrasound (USG) can sometimes detect features suggestive of harlequin ichthyosis.. antenatal USG has limitations in detecting harlequin ichthyosis, and a normal USG does not rule out the condition. So definitive diagnosis is usually made after birth

Mam pls reply -2sd ofc normal he kya? My baby boy 3month old and hc 37.8 ye kya normal he mam?dr ne head video xray karbaya Xray normal aaya
@learningpediatrics5347Ай бұрын
According to the World Health Organization (WHO) growth charts, a head circumference of 37.8 cm at 3 months is close to the 10th percentile, it's considered slightly below average but still within normal limits. However, it's essential to monitor the infant's head growth and overall development closely. Diagnostic criteria for microcephaly:- 1. Head circumference: More than 2 standard deviations (SD) below the mean for age and sex. 2. Severe microcephaly: More than 3 SD below the mean. Please consult a pediatrician for a comprehensive assessment and guidance.

Hello ma’am a doubt pls help Vit K is deficient in breast milk n babies at birth do not have stores n the gut bacteria takes 6 months around to make enough vit K Hence we give vitamin k injection to prevent hemorrhagic disease of newborn I was asked in my viva y not repeat the injection One more time I said may be since it’s fat soluble so not needed N second the gut will start producing but it would by 6 months So y do u not repeat the dose ? Any specific reason ?
@learningpediatrics5347Ай бұрын
Reasons for Single-Dose Administration of Vitamin K in Newborns: 1. A single dose at birth provides protection against Hemorrhagic Disease of the Newborn (HDN) for several weeks. 2. Newborns have limited liver stores of vitamin K, which the single dose helps replenish. 3. Repeated doses will increase vitamin k toxicity risk, especially in newborns with liver disease or underlying conditions..

If mesenteric lymphadenopathy is more than more than 2 cm child having recurrent abdominal pain without fever and weight loss. Is caseation necessary for tubercular lesions
@learningpediatrics5347Ай бұрын
A definitive diagnosis of intestinal TB requires a combination of the following: 1. *Clinical Features*: Presence of symptoms such as: - Abdominal pain - Fever - Weight loss - Diarrhea or constipation 2. *Radiological Features*: Presence of: - Mesenteric lymphadenopathy - Intestinal thickening - Ascites - Intestinal obstruction on imaging studies (e.g., ultrasound, CT, MRI) 3. *Endoscopic Features*: Presence of: - Ulcerations - Narrowing - Modular lesions on endoscopy 4. *Histopathological Features*: Presence of: - Caseating granulomas - Acid-fast bacilli - Inflammatory infiltrate on histopathological examination 5. *Microbiological Confirmation*: Detection of Mycobacterium tuberculosis in intestinal tissue or lymph nodes
