So, what happens to the "attraction" if the body changes afterwards, either due to accident or other factors

Well, even though physical factors are the most common sources of attraction, people often times find other mental / emotional basis for it. It is only in a case where attraction goes beyond physical factors that it is sustainable In the long run.
