We cannot give specific advice on your individual case because we have never examined and assessed your spine or specific misalignment and correction. That being said, assuming you have been completely corrected, we generally see relief from dizziness and vertigo respond quite quickly. Have you been checked and found to be holding your correction yet? It would seem to be a good sign that it responded initially but I would expect it to stay away if you are in alignment and holding your correction.

How many % of ppl with upper cervical misalignments ACTUALLY have EDS in the neck? Or it is usually because of the stretched ligaments due to the misalignments? If yes, how long does it take for the stretched ligaments to be toned enough to create stability?

@ttilles the majority of our patients have upper cervical misalignments without the presence of EDS. However, we do have some patients with EDS and they tend to do just as well in recovery as our other patients.

@ttilles an upper cervical misalignment is not the same thing as craniocervical instability. An upper cervical misalignment can also be referred to as the atlas subluxation complex. For more info on the ASC, check out our videos on it, part 1-3. CCI can be caused by ligament laxity in some of the ligaments around the CCI. This can be a result of a genetic abnormality or from a major whiplash trauma.

@ttilles we usually recommend holding off on most neck PT programs for at least the first month under care. After that, we encourage exercise programs of all kinds.

I didn’t have these synonyms before. what could be the cause? (Went to ER. Rules out of the scary stuff) My chiro doesn’t answer my questions like you.

@ttilles sorry to hear that you aren't getting your questions answered but the hard part about vertigo is that there can be a number of causes. BPPV is one of the most common and can be ruled out (and fixed) by a professional who performs the Epley's maneuver. We regularly see patients with vertigo and get them corrected and this takes care of it. Also, things like dehydration, medication reactions, food poisoning etc can all cause vertigo, among other things.

Generally speaking, after an adjustment, we will many times have some soreness and tightness of muscles as the body changes how we are standing. This is normal. In most situations, new neurological symptoms would not be considered normal.

can an atlas subluxation cause the dizziness, confusion, vision changes/light sensitivity effects that normally accompany occipital neuralgia? as well as crippling anxiety/depression, and inability to regulate heart rate as well as high heart rates? this has been the worst 7 months of my life. i went from having severe neck pain all the way down to lower back/sciatica that then progressed into the neuralgia/anxiety/increased heartrate thats hard to regulate, to now just the heart rate problem as well as my neck/upper back constantly cracking when i move. my PCP told me its all in my head prescribed me zoloft and a referral to a psychiatrist told me to go on my way since i never had any loss of motion in my neck shoulders or lower back just very slight pain when i turned my head to the side. weeks later i had a resting heart beat of 120-140 so i went to the ER in which they admitted me into the hospital and ran all the tests they can on my heart. in the end they told me all the tests seem fine but something has to be wrong with my heart so they prescribed me metoprolol and told me to check in with a cardiologist. the entire time i've been telling these doctors this started with the neuralgia and i wonder can it be an entrapped/irritated nerve (guessing vagus) one doctor confirmed its a possibility but their priority was checking the heart. i have no idea where to turn any more but this has been beyond debilitating in every way. i cant seem to find an answer and its really starting to scare me how much my heart rate jumps up when i stand and walk around when i dont take metoprolol and i cant stand the idea of me having to be on meds for my life because no1 can seem to figure it out

@ramsinbabakhan3629 your story is very frustrating but not uncommon in the least bit. Absolutely, an atlas subluxation can cause all of the symptoms that you described. Additionally, the fact that you had sciatic pain at one point tells us that your structure is compromised which tells us that your atlas is essentially guaranteed to be subluxated. You would benefit greatly from care. Let us know where you are located and we can try to help facilitate your finding a good option for care in your area.

Hey Doc: ever notice how often patients are holding the adjustment when the (CN XI) upper trapezius starts pulling? When it stops pulling, sometimes you can see that it’s improved the biomechanics if you re-X-ray the lateral view. Depends of course on absence of crushed segments and such which are actually part of the loss of curve. Sometimes the posterior VB line (George’s line) stepping can improve. I’m with you on this one. Let the nerve do their thing.

I have dizziness and ringing in the ears. I’m on my 2nd week of nucca treatment. I still have ringing in my ears. Some days, I have vertigo, some days, I don’t. Is this normal/retracing?

That being said, assuming you have been completely corrected, we generally see some relief from dizziness and vertigo respond quite quickly. If the dizziness/vertigo is happening less frequently, lasting for a shorter amount of time or is less severe, I would assume those are signs that it is headed in the right direction. However, the ringing in the ears (aka tinnitus) is a very different story. Sometimes it goes away instantaneously, other times it never fully subsides. And there are other cases that is goes away gradually. Hope this helps!

However, assuming you have been completely corrected, we generally see relief from nerve pain and neurological dysfunction happens quite quickly. This relief usually happens within the first two weeks and many times much sooner. We often see sharp shooting pain be replaced with a dull, deep achey type of pain. This is typically a good sign that the healing process is underway. Hope this helps!

Is the pressure always “like a butterfly?” Is it a red flag if the pressure used is more firm than that?

@blessingsinthemidwest730 some doctors use a heavier pressure than others, for different reasons. The big red flags that I would be concerned with are: 1. Is the doctor adjusting other areas of the spine? If NUCCA is done correctly, there is no need or benefit to doing this. 2. Did the doctor take specific pre and post x-rays (multiple if necessary) on the initial visit to get a very specific listing for your correction? This is a requirement for the work to be done correctly. 3. Is the doctor adjusting every visit? The goal should be to hold the correction for an extended period of time and require as few re-adjustments as possible. 4. Is the doctor using objective measurement data to determine if you need an adjustment on follow-up visits? Required to know when and when not to adjust. This is not an exhaustive list but this is a good place to start. Let us know if you have any other questions!

I have upper cervical misalignments I have a occipital and atlas misalignment and I have no pain but sudden insomnia and feeling fight or flight.. I’ve been getting adjustments and sometimes I feel way worse after adjustments it’s easy to have bad days and rare to have good days how long will it take and is it normal so get worse before better.. and it’s hard to determine if it’s healing when I’ve been feeling worse?

These are very good questions @leahann2655. The first thing we would want to know is if you are holding your correction for any length of time. You should be able to hold for weeks and then months. In our office, we do not adjust every time and most times, patients are holding and not requiring adjustments. Unfortunately, it is difficult to know whether what your feeling is related to healing or if it is something else without being able to assess your misalignment.

Hi @leahann2655, we appreciate the question and while we cannot comment on specific patients or their cases, we can shed some light on how a patient with sleep issues typically responds. A sleep issue is commonly a disruption of the parasympathetic nervous system and when we restore the function of this area the response is almost immediate. We see patients who are sleep deprived zonk out after their first correction in the resting room and then sleep for double digit hours that night and multiple nights afterward. As they hold their correction and their body heals, then their sleep cycle will begin to normalize.

it went in phases for me, but when i told my PCP during the pain all over phase i feel like im dying he laughed and said you cant die from back/neck pain. but the pain was bad, like a 10/10, it was so hard to manage it even with ibuprofen 800s. as for the neurological effects, i had sharp shooting pains down my arms to my finger tips and even my legs and toes. i was utterly confused, to the point id forget family members as they came off as strangers to me. any slight head movement led to the absolute worst dizziness ive ever felt, not that the world was spinning but that i can feel myself spinning across the world and with that came constant nausea. my vision was very hazy and foggy. i felt like i was floating as i was walking. thinking clear thoughts was nearly impossible for me and holding my head up was a task in itself. the 24 hour migraines that ran for 3+ months straight drove me to the brink of madness. it was during this phase i noticed crushing depression/anxiety, to the point where i was saying my goodbyes to the people i loved, i literally believed i was dying. with that, i started noticing tachycardia, my heart rate was 110+ sitting, id wake up to 130-140s and would have to lie completely still to get it to the low 100s, any slight movement would raise me right back to 115-120. i actually had to get checked into the hospital for my tachycardia and they still weren't able to understand why my heart rate jumped up when i never had this issue in the past. all my heart tests showed it was fine. it was at this point i started noticing erratic elevated bloodpressure when i was always on the normal spectrum in the past, i was completely healthy on a strict diet working out 7 days a week so this was very concerning to me. in the end i got released with blood pressure medication to help stop the tachycardia and erratic bloodpressure, and while it does help im still all over the map with my heart rate and blood pressure while even on the metoprolol. this has been the single worst thing i have ever dealt with, my PCP nor any doctor i dealt with in the hospital could understand what is going on. it wasnt until i was released i started finding these videos on CCI/AAI. i just assumed it was occipital neuralgia which i kept telling my doctors over and over, but neither ever thought to xray me or mri me which in hindsight i guess it wouldnt help them if i wasnt having Dynamic Digital Radiography done on me. i was lucky enough to find an orthopedic surgeon who is savvy to CCI/AAI but im having a hell of a time getting a referral from my PCP muchless knowing how long it would take me to see said doctor. at this point im mainly dealing with the erratic BP and tachycardia, chest tightness making it hard to breath even at lower heart rates, constant gas, neck soreness that keeps cracking everytime i move it including my upper back, some hip/sciatica pain that also shoots to behind the knee, heat intolerance and heavy fatigue. my vision is a lot less hazy but im seeing floaters everywhere, ive always seen floaters to a degree but its much more now.

I know exactly what you mean. It makes people like us feel invisible. We deal with so much more than Pain, numbness and tingling, some dizziness. I also stop breathing at times, experience fainting, extreme blood pressure fluctuations, paralysis, seizures, cognitive decline, elevated heart rate, and so many more strange symptoms that are so hard to describe. I do have instability of the ligaments as well, so I'm sure that's why my atlas is able to cause all this

Great question Nikki! Theoretically, it makes sense that an Atlas misalignment can cause increased CSF pressure in the cranium which could then increase CSF space in an area around the brain. Not saying that is always the cause, but it makes sense that it could be coming from an Atlas misalignment.

My neck was off by 1 1/2 degrees to the right causing migraines and jaw pain and incredible headaches Only by going to an upper ceevical chiropractor and X-rays did I uncover the cause Have done 3 adjustments so far and my neck is starting the healing process Only the upper cervical chiropractor actually gave me some hope My hip was causing me problems as one leg was shorter than the other and it was then causing me jaw problems I knew they were all related but I felt other professionals were not helpful I am so thankful for nucca and KZbin and videos like this for pointing me in the right direction

Or you feel temporarily better from the endorphin release from the "adjustment". This is the main hypothesis for why people feel better after adjustments as chiropractic adjustments dont actually move bones

Also, our pre and post x-rays prove that we move bones and align the neural canal. Our pre and post postural measurements, and follow up postural measurements show that the changes are not temporary. Not to mention all the functional changes. Again, thank you for the skepticism, it helps to keep us sharp. :)

Much appreciated Kalan great talk. I was wondering if I could get your opinion about prolotherapy have you seen much success with it to augment and maybe speed up the recovery process with NUCCA? I've had tremendous results with my chiropractor so far but still some strenuous activities throw off my alignment... it's been 15 months under care so maybe I just need to accept my age (52).

Appreciate the question Wes! As for prolotherapy, we have had a very limited number of patients who have used it. The results have been promising but I am not sure that they wouldn't have been had they just went the normal NUCCA route. Unfortunately, I do not have much of an opinion based on experience. In theory, it seems like it would be beneficial if some of the specific ligaments that are involved in the holding the atlas in position were damaged and/or the patient were having a difficult time holding after a severe whiplash injury. The other issue is the administration of the therapy and this is a whole 'nother can of worms.

@@trilliumspinalcare Thanks Kalan, yeah I've kinda come to the same conclusion not sure how comfortable I would be having needles inserted into my ligaments so close to the brain stem that can produce inflammation and symptoms for possibly several weeks... and it's pretty spendy (insurance won't cover it). I spoke with a gal at a clinic near me she said the doctor there was trained by Dr. Chris Centeno in CO. So I watched some of his videos and he claims he has the only reliable method for fixing craniocervical instability and anyone else is a waste of time/money. I know what does work so think I'll stick to it.

Ya that is a tough one. I am usually wary of any scenario where this is the "only" way to fix something. If you are continuing to hold, your body keeps healing, and your function keeps improving, I would say just to stick with it.

I can tell you that prolotherapy was the ONLY thing that worked for me, but I have Ehlers Danilos Syndrome and no matter how many times a chiro or DO would adjust my neck, it’d be out again in minutes. The first time I tried prolo, I gave it a couple of treatments and seemed to get better for awhile and then regressed. I thought, “well, it doesn’t last.” After suffering for a year, I went back and talked with them, and their take was that several treatments (I went to 16, total, once a month, to the tune of many thousands of dollars) would actually thicken up the ligaments to the point that they’d be strong enough to hold. It worked. In the four years since, my neck has not subluxed, and while I make periodic trips to my DO for other things, she always checks my neck but still declares it “perfect.” Prolo is expensive, it’s painful, and it’s scary the first few times, but being free from headaches, free from vestibular issues, free from TMJ issues, and free from the fear of cervical spinal cord damage after living with those things for SO long is beyond extraordinary. It’s like anything else, there are good and bad practitioners. I found a great one. It’s funny, I have spoken to other people with my disease who angrily declare that prolo can’t work for EDS because we have bad collagen, but I have strengthened my soft tissues through prolo and weight lifting (very careful and controlled) and am far more symptom free at 45 than I was at 30. Like many chronic health issues, many with it like to sit around and feel sorry fo themselves and get incensed when they see somebody else be successful.

Appreciate your question Lianne! An easy rule of thumb is that an adjustment is only bad if it is not needed. AKA if you are in alignment and you are adjusted that would very likely make issues worse and it is the reason that using objective measures to determine if someone is holding is so important. If you are out of alignment, an adjustment will benefit you no matter where you are in the healing process.

Nice explanations! I was watching an MMA fight and I was wondering: how can an adjustment be done by a small tap or push on the atlas while these guys fighting are twisting their necks like crazy and they are still healthy ? Not doubting bout the efficiency, i am under care and I ve seen incredible things happening so far but I am very curious. Thanks a lot :)

R R that is a phenomenal question! As with most questions that require deep thought, a multi layered answer is necessary. First of all, when a patient is in a position to be adjusted, their neck muscles should be relaxed. When an athlete is competing in a sport, their neck muscles are usually contracted and supplying additional support for the CCJ. This helps because there is only soft tissue holding the Atlas bone in position. This can be due to a multitude of reasons such as past injury, birthing process and complexity of it, size of the individual's foramen magnum in reference to neural canal, and many other factors such as amount of time spent in an aligned position along. Most NUCCA docs agree that certain "rules" need to be observed after the patient's initial correction to increase the likelihood that they will hold their correction. Lastly, sometimes we see someone experience an atlas subluxation in real time. They’ll pay eventually. Trauma causes damaged grey mater to dissolve and is removed by the cerebral spinal fluid. Then other gray matter takes it’s place and learns the function. An old person moves not as agile because they have run out of gray matter to replace damaged or aged cells;among other things. The cilia in ependymal cells also get tangled and can’t move the fluid as well….yada, yada. Cilia of ependymal cells line the central canal. Not yet studied thoroughly enough…..research funding for health care, unfortunately goes to other areas that are more profitable.

Is walking okay after an adjustment?

Thanks for the question Mim! We usually discourage walking after an initial adjustment because resting is much more important. However, more than anything it depends on what the patient's consistent activity level was like before their first adjustment.

That's a great question GC! In our opinion, the very best way to correct the ASC is done by hand, with an especially gentle force, using pre and post x-rays to analyze and verify an orthogonal alignment. Another way of saying this is the NUCCA protocol ;)

Not only is my anxiety better after a year of NUCCA I don't feel as bored, tired or depressed either. I still have bad days (especially if I don't eat very clean) but overall my general mood is much improved and I don't spend as much time playing video games and vegging out watching TV I'm trying to be more productive and also educate myself. Definitely give this a try if you are on the fence it's not a get well quick treatment (though sometimes can be) but a marathon that is worth everything to run.

Quick question, are you allowed to continue working out and lifting weights few days after an adjustment or will it push you back out of alignment?

Great question! Our general rule of thumb is to allow the body to heal for approximately 4 weeks after the initial correction before introducing any new exercises or workouts. For a patient to lose their initial alignment, it usually happens when someone feels stronger and more capable and pushes their body too far before it is ready. Maintaining the same activity level from before being aligned, for 4 weeks afterward, allows for our body to hold and heal.

Is there a pattern for retracing? For example, from the bottom up (feet to head) or from the inside out for example, stomach issues before skin?

Great question! What we see most commonly is more recent issues healing first and older issues healing after that. Also, when the body is healing and recovering in general, we typically see the healing begin more quickly in the axial skeleton before working to the extremities. Thanks for tuning in Lisa!
