looks like it hit an object while travelling - ambient fire tends to ignite objects, i used the everburn sword until act 2 and i kept lighting things around my target on fire when i attacked

If the projectile is big enough, even if the path is clear, it can hit something in the way and burst there.

Playing D&D as a monk I've found an amazing technique of getting enemy attention and then both me and the party warlock casting fireball on top of my character. It works really well.
