The scenario is flawed from the onset; patient presenting pneumonia-like symptoms should be evaluated primarily on the clinical presentation, we are also not getting adquate information to assess the need for a thorax x-ray. Is the patient hemodynamically stable, patients medical history, how are they saturating and so on. If the sats are over 90-95 and there is no immediate concern for anything acute, the patient is actually in the right in this case. Most you should do with the given parameters (stable patient, saturating ok and so on) is maybe order some basic bloodworks and infection markers to determine the etiology and plan out treatments if the symptoms become prolonged. Otherwise mostly good answers, I don't get the push for additional scans (no primary care doctor should ever order a chest MRI, literally zero indications in primary care setting AND the CT shouldn't be even considered in these clinical parameters, much less when the patient is worried about radiation exposure). Would the candidate get a perfect score for pointing out these flaws in the exam setting?

Doctor Abdul- As I was watching this I thought to myself maybe a simpler approach would be better. the longer you get into the explanations the more people get confused which can cause more issues. Would it not be better to say straight up to the patient to acknowledge that an X-ray is not 100% safe but let it be known that its the patients best shot at a good recovery. Or even take an approach where you let them know its there decision but make them aware of of the repercussions if they don't decide to go though with it.

The true reason why we cannot get GP appointments is due to lazy receptionists and GPs. A couple of days ago I did an e-consult, and I still have not had an appointment; reception is not answering the phone. Last month I also did an e-consult, some thing, no appointment made, though on that time I managed to speak to a receptionist, who waffled rubbish before she made an appointment after I said I might as well inform CQC. We could not get appointments when phoning for an appointment, or doing online. Now we have e-consult appointments only, we still are not getting appointments!

I got 2500 in 2022 which was the exact average score but got interviews from birmingham and keele, offers then from both. Will start at Keele in 2 weeks- definitely possible but looking back would have definitely applied to Sunderland too. acc worth mentioning I had a medical procedure a few days prior which meant I received consideration for that from bham, but keele did not consider that, still got an interview. Also reason I highly recc Sunderland if low UCAT is cos they like keele also value their R + R form more when choosing interview candidates

Doc, I have looked at what goes in to a vaccine, and the adjuvant used mostly, aluminium, is in there, and the thing about aluminium, it is a neuro toxin! And to think a new born is given the exact same vaccine that an adult gets concerns me greatly. A new born child, with an under developed immune system is given the same amount of sh*t that an adult gets. They keep taking on patient's. when they can't even see the ones they have they get there's two phantom gps .I've never heard such rubbish .his mum can't get a gp appointment . You can't even get to speak to a receptionist in mine . They send you down to a/e no wonder the there's queues . But the guv are complicit big time.
