They only speak Japanese because the creator of the show and manga is a Japanese person. Most anime are produced in Japan hence the Japanese dub being the original. There is an English dub as well. It's like when they have anime set in western countries they still have the original dub in Japanese.

From what I see, there’s some inaccuracies in their outfits. Like prostitutes dosent wear out their bosoms like that. And Eunuch have castrated manhood. The hairstyle and the tuck out top that maomao have dosent exist in ancient China, not event servant maids wear like her. And of course no anime pink hair.
