Men are responsible for divorce in ALL cases. Even if the wife commits adultery and wants Khula, EVEN THEN, it is the fault of the man because of three reasons: 1) He married the woman without inquiring about her Haya. 2) He didn't love her so much that he was sufficient for his wife 3) He did not teach his wife to become a good Muslimah.
