Excellent video, very instructive, here in a few days they are going to do a Femoral Popliteal Bypass Graft on my left leg, no getting a wire through the dissectional planes it looks like. I was unaware of the layers and being able to utilize that for access. I'm like an old car going in for new belts and hoses, just need a new hose this time, and an oil top off. Great info, great Dr. I have a question. I had a lower femerol bypass for left leg pain. A seroma occured, and they drained it. Then it came back, so I had a clean out. Now it seems to be coming back again. My question is how long does it take to go away. or...would I need another clean out? Theres no infection or redness. Its tracing the tube they installed.

In general, superficial thrombophlebitis is not life threatening but there are instances where you should be evaluated and followed closely. In rare cases, extensive superficial thrombophlebitis (a large portion of vein) may be associated with an underlying clotting disorder. Further blood tests may be needed along with serial ultrasounds. That being said, the majority of cases are benign, self limited and should resolve within a few weeks..

It depends if the thrombus (clot) is acute or chronic. Some patients might do well with aspirin therapy long term or even low-dose anticoagulation. There are several factors that would guide our decisions such as the cause of the clot, comorbid disease states, and the patients' overall health and mobility. Performing balloon angioplasty may also help open the stent as well.

I will be having this procedure soon and didn’t get a chance to ask this one question- what metal are the stents constructed of? For a person like myself who cannot wear metals other than gold, platinum or stainless steel due to allergic reactions on the skin, if I need a stent, sure don’t want one that could create other issues. Will be looking this up, but am curious if a patient has ever shown reaction to the foreign body/stent being placed.

Most self expanding stents are made of nitinol which is a mixture of titanium and nickel. The good news is that adverse reactions involving allergy to metal in stents seems to be very rare.

Do you pre-test your patients for metal allergies? I too am highly allergic to many metals, mostly nickel, and also have two autoimmune diseases. Anything foreign and anything foreign and an allergen - you can tell me it would be ‘rare’ for me to have a systemic reaction? Does the FDA include warnings of nickel allergies in the inserts of nitinol, self expanding stents being used today? 55% nickel and 45% titanium. Other than invasive surgery, if even that would remedy the compression, I will have to wait for newer, coated stents to be approved. As a well controlled type 1 diabetic, the very last thing I would want is to be on steroids for the rest of my life to battle systemic allergic reactions. Thank you for this informative video - I only wish a vascular surgeon could ‘guarantee’ I wouldn’t have issues, and I know this is an impossibility.

In 2019 I had a procedure done. My vascular surgeon did a right ilio femoral bypass graft. Day 7 after surgery I was taken down for a CT scan and Dr came to my room minutes after. A blood clot showed up by the graft. A few days later a Dr did the stent and removed the clot. Afew days after that I mentioned to my surgeon that the groin dressing from surgery was a funny color. He checked it out and found my incision had dehissed and was infected. Was put on strong IV antibiotics for a week. I finally went home after 21 or so days. Had nurses coming 3x week for a month to change dressings. I also had and still have a partial blockage in right thigh. This year in March I had an angioplasty done. The right ilio femoral artery had narrowed significantly just above graft and stent. The surgeon I have now went through my left elbow to get to right ilio. Just as he said, Cathy you're going to feel a bit of pain, I yelped and I was sedated but I was silly and automatically started squirming. A light dose of fentynol was given. I then slept on and off until they finished stitching my arm. Otherwise is was an easy process for me. I will admit I was terrified going in. I assumed that they would have to go through the heart from left side to get to right side. That freaked me out. Dr explained the procedures the best he could to allay my fears. Still a bit scary. Yes, you can perform most activities once you get home, however, I recommend patients take 1-2 hours to rest first as to avoid complications such as bleeding following phlebectomy. Avoid heavy lifting, but most everyday activities are allowed.

I read somewhere the Anterior accessory saphenous vein is the real culprit. Why do you do ablation on the GSV? Also what happens if you don’t get the entire vein out? If I lay down my veins are barely visible assuming gravity but if you cut off the blood flow like you said upper thigh wouldn’t that mimic laying down? So is cutting out necessary? Any risk of nerve damage? Thank you if you are able to respond 😊

Hello, if there is an anterior saphenous vein, yes we will treat it if we think it is contributing to your symptoms. Many patients have insignificant anterior saphenous veins and do not cause symptoms or varicose veins. As a general rule, we always treat refluxing great saphenous vein as they will ALWAYS communicate with the anterior saphenous vein. And yes, taking care of the entire vein is helpful especially if there is a large varicose vein associated with it. Based upon your description, you need more treatment and the anterior saphenous vein may be the causative vein or perhaps, the distal portion of the great saphenous vein (below knee) may need treatment.

@@mvitv1693 my stenting procedure had been done here in Italy, Milan. My vascular surgeon also didn't insert a guidewire in my right iliac vein to better understand the position of my vena cava so the stent doesn't go far into the vena cava.

May-Thurner Syndrome is a physical compression of the iliac vein so anti-coagulants will not solve the problem. Stenting is needed to open up the vein and keep it opened.

The recurrence of symptoms such as pain, swelling, or discomfort in the legs may prompt further evaluation to assess the need for additional interventions. It's important for individuals who have undergone GSV ablation and experience recurrent symptoms to consult their healthcare provider for further assessment and management.

Good video but something less than complete list of alternative treatments. I corrected arterial plaque with zero intrusive procedures, and zero side effects, naturally . I had blood clots in my leg and when I had a follow-up ultrasound, my clots were gone but they seen something, so I had a ct, and they seen a block artery, I had a more clear ultrasound at a vascular surgeon, seen my blockage and narrowing, it was clear. They said I was down to %60 blood flow to my right leg. My leg changes horrible color's when I stand, and extremely painful. I have a follow-up appointment next week, what should be the next step. I am on two different blood medications, one a blood thinner and another for the block artery. I also had knee surgery in September in the same knee. A stent has been mentioned more than once.

I'm also having these cold spots throughout my leg, making it feel wet, really cold. I am on blood medications but it's getting worse. First it was my foot turning purple now it's my whole leg smh. Hard to tell what's what because of the patella reconstruction surgery, blood clots, blocked artery. They all cause pain and swelling. I see my vascular surgeon on Friday. @mvitv1693 I just wanted to update, I seen my vascular doctor and I have a blockaged in my iliac artery along with narrowing so we will be going in to put a stent in. However it will be going in from both sides so it can remove all the block. I guess I will be staying overnight so they can watch me, because of my high risk of blood clots. I'm ready to get it done so I can move on with my life lol thank you again for your video, it definitely helps us not be so scared 😊

Typically for procedures such as these we use a combination of medications through an IV to keep you comfortable. It is termed conscious sedation, so the goal is for you to be conscious and arousable throughout the entire case, but comfortable.

I need a completely occuded anterior tibial artery re-vasculated . An Angiography was performed May of 2023, with the artery occuding after only 3 months. No stent, only ballooning. (Two stents were placed in the upper leg). DO ANY OF YOU GUYS COME UP TO RHODE ISLAND?? I NEED A NEW SURGEON!!!

Smaller vessels in the lower leg are typically treated with ballooning rather than stenting due to the size of the vessel. Factors such as genetics, diabetes, elevated cholesterol, smoking, and lifestyle can lead to these vessels re-occluding. It is important to walk/exercise daily, quit smoking if applicable, manage blood sugar and cholesterol levels and take medications as prescribed to try to prevent this. The long term treatment is based on the intervention performed. Balloon venoplasty will not necessarily require any long term medication. Most stenting requires at least short term antiplatelet therapy, but this will be based on your physician's recommendations

What do you all consider a severe blockage? My moms doctor never gives a percentage just says severe, she’s already had one amputation, bypass, graft and stent, they want to perform an experimental surgery on her other leg and we almost lost her several times and I’m just scared for her. No legs doesn’t leave her with much quality of life after several strokes that has made her weak so moving by arms isn’t an option, she also has been diagnosed with vascular dementia. A lot of this has happened right after we lost my brother and nephew, her son and grandson. It has caused so much stress on me that I had a massive heart attack March 2023. I worry about this everyday! 😢

We are sorry to hear this and wish you the best. To answer your question, the severity is really determined more from the symptoms and physical exam rather than a percentage of stenosis. A severe stenosis would cause pain both with ambulation and possibly at rest and may be causing discoloration of the skin, wounds or tissue damage. Best Regards.
