Go to any small isolated community and there's a fair bet that you'll find a lot of cousin marriages, even if the partners might not realize that they're related. My own family on one side lived in one small village for hundreds of years and I wouldn't be surprised if I'm related to most of the people in that village even today. On a more serious note, what about Iceland? Geographically isolated and hence bound to interbreed, but they don't seem to have suffered from it.

Interesting that the very strong American taboo against inbreeding is reflected in the defense nature of the comments. As an American I was feeling it myself, because cousin marriages, etc. is linked to class. You are low-class, ignorant, uneducated, white trash, etc., if you’re marrying/sleeping with your cousins. It’s a joke that poor whites in the Southern states are inbred, and the people who live there can be sensitive about the stigma and prejudice. Part of the reason for this is the vast size of the country, with certain geographic population groups being very isolated. Access to other populations can be difficult in certain areas of the country. You live in the mountains, you marry people in the mountains. They might not be actual first cousins, but over time the genetic commonality of a group increases. People in Europe have no idea about the size of the USA. To drive across the country is the same as driving from Moscow to Lisbon.

Child mortality was not unusual by any stretch in those days. It was the norm actually, not unusual for people to name children the same name. Not saying the inbreeding was not the cause of death, but based on this I have my doubts they were.
