Many thanks for sharing this educational video. Just to emphasise- in hemiballismus, the lesion is usually in the contralateral subthalamic nuclei. BUT it can also be caused by lesion in the afferent and efferent pathways of the subthalamic nucleus, corpus striatum, thalamus or parietotemporal cerebral cortex too.

Is this coming from the pterygoids? (noticing side to side motion of jaw). Mentalis muscle also seems to be in spasm (lots of dimpling). Would botox give some relief? Mentalis is commonly injected for geniospasm. Is there synkinesis between mentalis and pterygoids? Anyone know?

13 years ago... the FND dark ages, when I was first dismissed and/or outright told I was purely having a psychological issue of unknown Somatic origin with my drop seizures by three major health systems. From the dark ages when a leading, well-respected, Head of Neurology at a hospital system directly said to me "I don't know what it is, but you are still alive so just keep doing what you're doing". While I had cast on the arm they had just put a plate in from a seizure, I might add. I was referred to another second seizure study - which by accident of my geolocation was at Stanford and overseen by Prof. Robert Fisher M.D.. Although atypical (older male) for FND at that time as a patient - Doc Fisher went out of his way to present my diagnosis in a normal manner, did not "weird out" and promptly brought in a neurological and psychosocial team to move forward with treatment. After years of DBT/CBT coupled with PTSD and relevant treatment, I have a life again. Neurology is a bit of a odd specialty. So often it is about managing and observing the decay of a patient, from my viewpoint. I think many neurologists may not have the perspective that real, tangible gains for patients are within reach, frankly. Sigh. FND does have a very real treatable positive benefit in a lot of the cases I've seen in my decade-plus. I'm a rabid promoter of Prof. Stone's website. I had the distinct honor of hearing him speak at Stanford almost a decade ago. And am very happy - no - elated that this field has expanded and grown in awareness, depth, and effectiveness. There is a lot, as in all human progress, still to do. Thank you all for being a part of this, and to all with that curious mind looking for solutions to your existing or future patients, my gratitude as well. Indulge me please with a few observations I've made over the years; In treatment please do not reward the seizures or symptoms intentionally or focus on them rather than attempting to treat the larger issue. I've seen FND folks get "rewarded" for behavior and it concreted the symptom (s) in their heads. This is, suffice it to say, not good. Please also attempt to communicate this to partners = parents = friends and family. Yes, treat these issues, by all means. But endeavor instead to change the operative conditioning that may be at the root of the problem. Secondary to this but no less important, try to place an emphasis on relevant social and psychological treatment. Just CBT/CBT groups without individual psychological is not very effective. They go hand in hand and should not be separated. In my opinion, I'd say that if you can just provide one or the other, perhaps neither as a single "stand-alone" service then perhaps "none" would be a better choice. Your patient and their resources can really be the determining factor here. Just a point to consider. The number one recommendation, the single biggest factor in my experience leading to having a life with this is undoubtedly having the patient meet others with FND. Breaking the isolation - the acceptance and knowledge that this is a "real" condition, they are NOT insane, deluded, imagining things critical. It is a "normal" condition, and a lot is known about it. Yes, it will be hard to treat (chemotherapy for the brain as I refer it ) but it can and will improve given effort. Also, understand that in the USA proof of aliens is undisputable. They all exist in Medical Billing. Medical billing still communicates in Cuneiform. EVERY ONE of your patients will have significant problems with insurance - payment and treatment resources. I have yet to confirm this worldwide but I have my suspicions. Finally, the multifarious horrible of KZbin's multitudinous clamor on FND can be intentionally shortsighted, woefully out of date, or just plain ass stupid. I do not encourage, advocate, or routinely recommend anything but specific videos from specific providers. You know this with any time spent with a "KZbin-diagnosed" patient. Please warn them that down that path leads to danger, and confusion and refer them to specific information. The KZbin algorithm is not your friend with FND. In my FND community, this is the single biggest issue I personally get asked about constantly. Granted, friends, partners, and family are truly well-intentioned. However, the abundant glial cells are still not firing in the KZbin body of work. Do please endeavor to directly educate the patients' social circle if possible. As well as your peers and coworkers. In conclusion (Whew! they thought...) I personally want to thank you for being the kind of person to care about this terrible condition. It is really a difficult life-changing experience and your learning more, from a reputable source, is admirable.

After 10+ years of seeing at least dozen neurologists and been prescribed 30+ drugs are ALL HAVE BEEN PAINFUL awful! The injections took a year to wear off ! I give a new drug no more than 6 doses ! Most don’t work after the 1st
