A great technique in experienced hands. I personally prefer I/A, but I have seen many Brazilian surgeons using this technique and overall I felt that their complication rate is extremely low despite this technique and a high percentage of advanced cataracts. The cortex and I/A free style has come about to save time and cost as some of the high volume surgeons perform over 100 procedures a day, making them so experienced and routined with over 15k procedures a year that their colleagues in other parts of the world just can't believe it until they see it with their own eyes. Using I/A in every surgery would make it hard for them to meet the demand for cataract surgery in their regions. Of course, economic reasons also play a role, as not using I/A reduces costs significantly, especially in these high volume settings. Definitely great and interesting but I still wouldn't advise to try it. 😉✌🏼

Phaco surgery is art , is medical practice not something to show off in it or even compete in it , just try to do as much as uncomplicated cases you can

Is DIB00 with laser surgery worth an extra $4500 above what Medicare pays? My dr is not as forthcoming about explaining the pros and cons of the lenses. My doctor says no distance improvement with the ZCB00.

There is no justification Would get burned if things went south and got sued No medico-legal board , M&M committee and anything similar would let this slip if PC occurred It would be considered reckless. We don’t skip the standard practice because we are too good for it Good surgeons improvise at necessity , not at will .

Is it much different than the Scharioth IOL piggiback IOL for macular problems? I mean, despite the obvious technical differences such as foldable versus not foldable, that one being a piggiback IOL and this one the main IOL. The Scharioth IOL is the equivalent of a +6 for reading very big letters in 15cm distance (I don't remember exactly)

It’s my standard secondary IOL with a very similar technique and has been for about 15 years. Inserting in front of the pupil is much harder and I would not recommend. In vitrectomised eyes an infusion makes things safer. Now I understand why sutured lenses are so popular in the US - it’s because you don’t have these!

Regardless of how experienced a surgeon is, the tissue should be respected, especially the posterior capsule.

For whom are Against this technique, why not? sometimes during phaco surgery, the pupil gets very narrow, and removing cortex with I/A probe would be risky, so why not getting rid of it at the beginning of the surgery

The fact that he chose a patient with a trifocal IOL (eye 100% healthy) to do this.... It's ALMOST bad practice. Because, at least with a patient with previous patology (glaucoma, macular problems) the patient has "less to loose" in worst case scenario.

Removing cortex with phaco. Advantages: - "I made this beautiful surgery video, look how skilled I am, I'm a pro 😎 " -I saved about 2 minutes of my lifetime Disadvantages: -Much higher chances of PCR which may result in more chances of retinal tear, retinal detachment, cystoid macular edema, endophtalmitis.

It’s dangerous even with high dexterity. I can call this « excès de zèle " Would never do it. Dr Neto , mohanta… all had capsular tear with this technique.

I’d prefer to select my surgeon and trust him, every move and analysis of my surgical plan is being considered. I had a lot of cases that i know is not safe to ( incision leaking, instable anterior chamber… ) and i just switch to the A/I and finish the case. Always good to hear different opinions.

👏 Very nice technique. Pop up lens partially and put a spoon behind it so it doesn't fall back. Sometimes tough to get the whole lens up and out of the bag if the rhexxis isnt big enough so I can see when it could be helpful !
