True exfoliation. Seen it 2 or may be 3 times..just carefully go about the rhexis

...it's very safe, no visco use. In a poor area, like my country, WE use charleux canula (23G) during our MSICS to perform thé rhexis on that way. it's reduce thé cost....

What an amazingly skilled surgeon...but as a beginner practitioner I would question myself several things...what advantage will this technique have over a conventional cystitome or forceps? Is it faster/safer/more neat? Is this technique only applicable for intumescent hypermature cataracts? Unless i get clear answers to these questions in my head, I will definitely not try to master this, until I have done at least 50,000 more cases!!! Thanks for the video though sir

The question is, would you do this to yourself? There is no need to perform the capsulorhexis this way.

Could see useful if lots of lens milk but would take a lot of cases to trust the vacuum

Amazingly but crazily done. I prefer less risks for my patients by using forceps.
@mbbssauag2 күн бұрын
Innovative .. but definitely not repeateble ... associated with its own set of risks... simple cystitome or capsulorhexis forcep are much better and efficient alternatives and are time tested ...

It will need super control over the foot pedal. Even a small increase in aspiration rate will dangerously affect the lens bag complex. Though nicely done by the operating surgeon, a big NO from my side.

Excellent video as usual from Dr Devgan. Really unexpected statistic that 90% of the experts would choose a monofocal over a Vivity. Why would anyone choose to opt for Vivity?

Apologies for posting on this old video but the subject is of great interest to me. I have a very very amblyopic eye from birth. I have cataracts so need to have surgery. I’m wondering if there’s any point in getting anything other than a standard monofocal in my lazy eye? I’m thinking of getting my lazy eye done first followed by my good eye a week or so later. I know this is the reverse of what Dr Devgan recommends but I feel happier going this route. Would be interested in any comments people have particularly regarding choice of lens for lazy eye.

It is interesting to know if also the posterior part of the capsule is double too and what precautions to take in the future in case when a PCO develops. Also it would be interesting to know how is the capsula in the other eye, in case it will require also cataract surgery in the future. The patient should carry with him a document containing his past eye history, when facing another eye surgeon in the future. Interesting case, beautiful video of a very successful eye surgery. Well done.

The anterior capsule naturally has 2 layers... therefore the "tram-track" sign of a normal rhexis... it has happened to me once, but unfortunately there was no video recording...
