Hi Jebin, if someone has had a fall but responds to verbal commands, do you score them 3 or 4. What are some of the examples that can make you score a patient 3 Thanks

Hi ! I am not sure that your query is about GCS or Braden . As you raised this query under Braden I guess the below explanation will help you for better understanding. The first parameter is Sensory perception. That is the ability of the skin to respond to pressure related discomfort. Then How the conscious level is related to this is ,if the patient is fully conscious I mean a normal person ,of course there will be no impairment in the sensory perception. As the Conscious level decreases the sensory perception also decrease So in case if some one had a fall and they don't have any problem in the sensory perception ,you can give a score of 4 May be the next day due to fall if something happened like the patient become slightly confused then you can give a score of 3. The very next day when you assess the patient becomes drowsy and responding only to painful stimuli then you can give a score of 2 The score varies based upon the response of the patient. Thank you
