01/06/2011 Dr. Jeremy Barnett The Emergency Registrar on Duty Maroubra Hospital Lakes Road Maroubra Dear Dr. Barnett, Re: Mr. Jone Elvin, 48 years of age I am writing to request your urgent treatment and assessment regarding Mr. Elvin whose features are suggestive of acute myocardial infarction and exacerbation of asthma. Today, Mr Elvin presented complaining of sudden onset of crushing chest pain and audible wheeze. His chest examination revealed bilateral crackles and slight S3 sound. His ECG shows mild ST elevation in anterior leads. GTN patch, IV morphine 5 mg, ipratropium bromide 500 ug via nebulizer and furosemide 40 mg were given. Initially, he presented to me on 05/05/2011 with complaints of exertional chest pain. However, his examination findings were normal, along with a normal ECG and troponin level, and he was planned to be reviewed in a week with reports of his exercise stress test. He has a history of asthma, for which he takes Seretide and a salbutamol inhaler. He is a smoker and consumes alcohol regularly. On review after a week, he was still symptomatic, and his test showed slight ischemic changes. In addition, his symptoms of asthma were aggravated, probably due to infection. He was then commenced on Lipitor, nitrates, aspirin and Anginine. On subsequent review, he was found to have persistent chest pain and bilateral wheeze. It seemed Mr. Elvin has not been complaint with his medications, and his alcohol abuse is likely to the reason. I have educated him regarding the importance of the medications and have also advised him for alcohol dependence treatment. In view of the above, my provisional diagnosis is acute myocardial infarction with exacerbation of asthma, and have requested a paramedic transfer. In addition, she had blurred vision with edematous eye lids and conjunctival congestion. Thereafter, chloramphenicol and Bion tears were prescribed. Ms. Maxwell has had thyroidism for 3 years, hypertension for 1 year and a hip replacement was done in 2009. Her current medications are Thyroxin 1 mg, Atacand 4 mg and Fosamax 10 mg daily. She has no known allergies. In the view of the above, I believe she needs immediate eye care facilities. I would appreciate your urgent attention to her condition.
