Dealing with ankle injuries can certainly be frustrating, especially when you have specific fitness goals like running a 5km. Steroid injections can be an option for reducing inflammation and pain in the affected area. However, it's important to weigh both the potential benefits and risks of steroid injections, as they are not benign treatments. Benefits might include rapid relief of inflammation and swelling, which could help you get back to training sooner. On the downside, potential risks may include weakening of the surrounding tissues, potential increase in infection risk, and other local or systemic side effects. Given that you have an upcoming running commitment, it's essential to discuss this treatment option thoroughly with your doctor or a sports medicine specialist.

Dr, I work on boats, I had a rough day on the water, boat bouncing a lot. At the end of the shift I couldn’t walk. I have severe pain in the front of the knee, and on both sides. Twisting it was Severe and made me pass out, on the sides is extremely tender to touch and a lot of pain and pain right on the knee cap. MRI showed no tears just patella maltrscking but I got worse after a week of resting and icing and I now completely can not walk. What could this be ?

Unlikely to be anything significant. But still your symptoms of not able to walk is concerning and I would recommend checking with a doctor to make sure all ok.

Qeustion can i take saxenda on a lower dosse like 0.25 or less? Do i need to start at 0.60? Why is this higher start then ozimpic? Thank you
@srikanthnithyanandammd
@srikanthnithyanandammd 3 күн бұрын
Each medication indeed has its specific dosing guidelines, including Saxenda. For Saxenda, the starting dose is typically 0.6 mg, which is part of its dosing regimen to gradually increase the dosage to minimize side effects and improve tolerability. Starting at a lower dose like 0.25 mg isn't standard practice, and due to the design of the pen injector, it may not be possible to accurately dose lower than 0.6 mg. The starting doses are determined based on clinical trials and the drug's pharmacological profile, which might explain the difference in starting doses compared to other medications such as Ozempic. It's crucial to discuss your dosing questions and any concerns with your doctor, as they can provide guidance tailored to your health needs and ensure you're using the medication correctly for the best outcomes. Always follow your healthcare provider's instructions for any medication.

Hello Doc. While jumping I had a innerward bend in my knee. From the pain and symptoms it seems it's MCL sprain grade 1. It's been a week, I can walk, I limp sometimes in between and have a problem in straightening my leg or bending at it's full. I'm having my Army interview next week. What am I supposed to do? I need to perform jumps and runs there.

I have stage 3 osteo arthritis in the left knee and stage 2 in the right and am considering stem cell therapy. My question Doctor, is if I get a hyaluronic acid shot, can I still try stem cells later on? if so, can it be done soon after the h/a shot? in other words, are they a compatible therapy and could be done together? Thanks

While I don't have extensive experience with stem cell therapy, I am familiar with PRP (platelet-rich plasma) treatment, which is somewhat analogous. I have no objections to performing PRP procedures following hyaluronic acid (HA) injections.

Thanks for your video Doctor. I met with car accident three months back and still taking bed rest. My both bones fractured below the knee. I consulted with the doctor and used knee brace for two weeks. I took mri 4 weeks after the accident. Mri shows fracture and acl tear. But I never have ACL symptoms. But after one month of accident I started to feel MCL symptoms. While walking with the walker inside the house i feel pain in mcl area. I have all symptoms of MCL..But MRI did not show MCL. .Now I sm walking with the walker without putting weight in one leg to avoid pain. How to recover from MCL pain doctor.

@srikanthnithyanandammd
@srikanthnithyanandammd 19 күн бұрын
Given the circumstances, it's wise to consult with a healthcare professional to confirm the severity of the injury. If it's indeed a Grade 1 MCL injury, a period of relative rest, typically ranging from 7-10 days, is often recommended before gradually starting exercises tailored for rehabilitation. This approach aims to prevent further strain and encourage proper healing of the ligament. However, the specific recommendations and rehab exercises should ideally come from a doctor or physical therapist, as they can offer guidance best suited to her specific condition. Wishing your wife a speedy recovery!

Hello I strained my MCL 6 days ago but I don’t know how. I believe it’s a grade 1 since I am able to bear weight on the leg and can do slow full ROM squats with balance assistance. Initially, I felt a pulling sensation when I would bend my knee but it went away around the 5th day. Should I start at phase 1 and continue until the end of phase 2?
@srikanthnithyanandammd
@srikanthnithyanandammd 19 күн бұрын
Given the absence of a clear mechanism for the injury and the symptoms you're describing, it might be something other than a straightforward MCL strain - sometimes conditions like pes anserine bursitis can present similarly with the symptoms you've mentioned. It would be wise to start with phase 1 of your rehabilitation to safely gauge how your body responds. However, it's also very important to get a professional evaluation to ensure that you've correctly identified the issue and that everything is indeed okay. A doctor or a physical therapist can provide you with a precise diagnosis and guide you on the best course of action for your specific situation.

For spinal lumbar Disc herniation ,which popular exercises do you recommend ?
@srikanthnithyanandammd
@srikanthnithyanandammd 19 күн бұрын
For managing spinal lumbar disc herniation, extension-based exercises are often found to be beneficial, but it's important to remember that the effectiveness of these exercises can vary significantly from one individual to another. It's key to base the choice of exercises on how movements affect your pain. If certain movements exacerbate the pain, it’s best to avoid those and focus on the ones that feel comfortable. However, I must emphasize that any exercise regimen should be patient-specific, tailored to your unique situation, and ideally, developed in consultation with a healthcare professional.

Hello, As far as I know , Heat is contraindicated when you have acute cases like acute pain in the joints due to osteoarthritis or acute bursitis , or acute tendonitis because heat increases the blood circulation to inflamed tissue and due to venous stasis you get more swelling and worsening of the case. I just heard this , please correct me and let me know what do you think about it.
@srikanthnithyanandammd
@srikanthnithyanandammd 27 күн бұрын
You bring up a commonly held belief regarding the use of heat in acute cases. However, recent studies suggest that the concern for increased swelling due to heat might be overestimated. While it's important to approach each case individually and with caution, the idea that heat therapy exacerbates inflammation isn't as clear-cut as previously thought.

Jogging is a higher impact exercise than walking but must consider that jogging covers more distance with each stride than a walking step. The critical number is cumulative load. And the difference is not that significant. So bit misleading to say jogging is higher impact. If we have a 100 pound person walking 1 mile: 2000 steps X 250 lbs per step = ~500,000 lbs cumulative load. If we have a 100 pound person jogging 1 mile: 1600 strides X 420 lbs per stride = ~640,000 lbs cumulative load. Do you agree? 🤔
@srikanthnithyanandammd
@srikanthnithyanandammd Ай бұрын
You raise an excellent point about cumulative load, and it's crucial to consider both aspects when discussing the impact of different exercises. Indeed, jogging does involve a higher peak joint force, which can lead to concerns, especially for those new to higher impact activities or with pre-existing conditions. Your calculation beautifully illustrates that the difference in total cumulative load between jogging and walking isn't as significant as one might think. However, the higher peak forces in jogging cannot be overlooked, as they are often the culprits behind the increased risk of injury if a person hasn't properly conditioned their body for such impacts. In my clinical experience, I often see patients start developing worsening symptoms if they do not progress slowly;
@Better_Call_Raul
@Better_Call_Raul Ай бұрын
@@srikanthnithyanandammd Yes, my doctor told me to walk (diagnosed with knee OA and normal BMI). I asked him if I could jog. He said to listen to your body. So he did not restrict anything. In my experience, walking does not worsen symptoms. I typically walk two miles a day and rarely have joint pain the following day. I recently tried mixing in a 1/4 mile jog as part of my 2 mile walk and experienced moderate joint pain the next day. Expect that the joint pain will mostly resolve within 24 hours. Will continue to monitor. My goal is to jog 1 mile and walk one mile. I think it is possible with gradual introduction.

Hello sir , I had an accident 5 weeks back and got my MCL injured ( Grade 2 on MRI ) and complete MPFL tear but on clinical examination kneecap was not dislocated according to my orthopedic surgeon so surgery was not recommended for complete tear of MPFL . From last 5 weeks I’ve been using hinged knee brace continuously but had no exercise. I was on rest and now I can’t fully extend or flex my knee which is making it difficult for me to walk . Please make it clear for me from which exercises do I’ve to start and for how many days should I continue it ? Should I wear my knee brace during exercises or not ? And until when should I leave wearing a knee brace ?
@srikanthnithyanandammd
@srikanthnithyanandammd Ай бұрын
Most of the time, the course of treatment depends on the clinical examination. If you've experienced a patellar dislocation, there's a chance you might also have a slight MCL sprain. However, the primary focus should be on healing the patellar dislocation. In cases involving an MPFL injury, I typically advise two weeks of immobilization to allow the ligament to heal properly. After this period, it's beneficial to begin slowly increasing the knee's range of motion. If it's been five weeks and you're still unable to completely extend or flex your knee, I highly recommend consulting a therapist. This situation calls for specialized, one-on-one rehabilitation to prevent the development of knee stiffness, which could lead to long-term issues. I hope this advice is beneficial.
