In general it shouldn't because you are measuring movement of the femur relative to the pelvis, so pelvic position shouldn't affect your measurement. In theory it is possible if the lateral tilt is large enough that it might affect the resting muscle position/tension for muscles that cross the hip which may impact the overall motion. Grade 2 is done in a gravity neutral position, sidelying. To test hip flexion the clinician must support the weight of the leg so any potential weakness in the hip abductor muscles does not interfere with the ability to flex the hip. If the person can move through full range of motion in the gravity neutral position they are considered a grade 2. Thanks for watching and we hope our response is helpful.

In this video we chose to describe each movement individually to improve learning of the basic techniques for each movement. If combining them during an actual treatment then you can refer to the diagonals by their D1 and D2 designations with D1 including anterior elevation-posterior depression and D2 including posterior elevation-anterior depression. During our range of motion videos we wanted to put the emphasis on learning the specific alignment landmarks and proper technique and thought inclusion of a specific measurement might interfere with someone from remembering the normative ranges for a particular measurement.

Is the goniometer backwards? Doesn't the mobile arm go down the finger, and the stable arm go down the back of the hand?

With goniometry, because the two arms are moving relative to each other it doesn't matter which arm goes on which body part as long as the angle can be properly read. It is very important to note the reading before movement starts as well as the reading at the end of motion. The difference is the amount of active range of motion. In an ideal set up, the goniometer reads zero before movement to make the measurement easier to identify. Thanks for watching and we hope this is helpful.

While moving towards flex or extension the skin also move, how accurate would be this measuring techniques?

This is a technique used primarily to document changes for a particular patient during the course of treatment. Motion of the skin may certainly impact the overall measurement, but since you are comparing initial measurements of a particular patient with future measurements of the same patient then the overall impact of the skin should be minimal. If the patient undergoes some significant change in body weight or suffers an injury that impacts superficial fascia and skin mobility then this technique would not be the best option. It would not be recommended to use this technique to compare measurements between different patients due to differences in skin/superficial fascial mobility and body type. Currently there are no agreed upon normative values for the general population with regards to thoracolumbar lateral flexion using the fingertip to floor method. However there is some early research working towards identifying some standard measurements using a tape measure. Here is the study from 2022 for your reference. Patel, P., & Parmar, L. (2022). Normative values of trunk mobility in normal adults. International Journal of Health Sciences, 6(S4), 6171-6179.

Positive shoulder adduction range of motion wound require a movement of the arm past the midline of the body. Because the trunk gets in the way of the arm moving past the midline of the body, there is no pure adduction range of motion to measure. Basically shoulder adduction in the frontal plane is considered just a return to the anatomical position from and abducted position. For movements across the body, horizontal adduction can be measured. Thanks for watching and we hope that is helpful.

When assessing a motion like thoracolumbar rotation that involves multiple joints and soft tissue structures we don't document end feel. If motion is limited, then a further assessment of the mobility of individual spinal segments may identify a particular segment or segments that have abnormal mobility with a specific end feel. However in order to know whether the patient with limited ROM ,what are the measurements to get to know, including normal motion for this movement using measuring tape. There should be a readings to differentiate. Thank you..

There are no normative values for this method of measuring lumbar extension. There are normal values using other methods including goniometry and inclinometers. If you choose this method you would be using your measurement to document changes for a specific patient, but would be unable to compare this patient to others. How do you get a foot to flatten out ? Does a stiff big toe contribute to the arch not flattening out? Or do the go hand in hand? I have insertional Achilles problem on one foot and my chiropractor says its a stiff midfoot. Any suggestions?

It is impossible to provide any specific guidance without a full evaluation, so we would recommend finding a local PT that has experience treating feet. In general terms it is common if you have a high arch to also have some involvement of the big toe, usually the big toe is not the cause of the arch problem. Good luck.

As a general principle you perform manual muscle testing to identify muscle groups that are impaired. That information must then be used in the context of the overall status of the person and their injury/illness to identify a level of exercise that is appropriate. The manual test results should not be the only factor considered when choosing the proper exercise. Once all information has been gathered then appropriate exercises are identified. So... what if you cant kick your legs out to the side at all on one side and maybe 10 degrees max on the other? I can fake create internal rotation and in fact did it during testing to force more movement by essentially rolling my weight off to the side of the hip rather than centrally.

One of the principles of all manual muscle testing requires that the physical therapist watches the person for any substitutions during testing and corrects it. Eliminating compensatory movements helps to ensure the manual muscle testing is as accurate as possible. Thanks for watching.

Yes phenomenal. One question please. Does it mean that midstance or a transition to midstance is a combination of lateral rotation of the hip joint and internal rotation of the ankle (pronation)?

At midstance the hip is transitioning from internal rotation to lateral rotation as the opposite side of the pelvis moves past neutral and backwards. As the foot approaches midstance it transitions to a pronated position to unlock and adapt to the ground. As we near terminal stance the foot returns to a supinated position to create a rigid lever to propel with.
