Nice discussion Elliot. An additional factor is the complexity of cross sectional imaging that has exponentially exploded. When I was your fellow in the mid 80’s, a CT had three, maybe four sheets of images. Now, a routine oncology multiphase CT chest/abd/pelvis (sometimes neck too) with comparisons requires reviewing thousands of images, (typically 1.25mm slice thickness or less)…including the current case to look for findings, and then compare to the priors, sometimes several. The older the patient, the more the findings. Sometimes there are 50 or more findings per case and if each takes at least 30-60 seconds to assess, compare, dictate, you’re looking at a productivity black hole. It’s no surprise the backlog is so great. It’s also not surprising that when radiologists are paid by the click/RVU (such as private equity owned practices) accuracy/sensitivity (from what I’ve seen) seems to crater…a topic of academic interest?

I was a resident "back in the day" when jobs were scarce for rads coming out of training (mid-90s; "look right, look left"). Now I'm "the old guy" and looking to retire in a few years. Radiology has been a great career, but 30 years has been plenty. As you allude to, it's not just radiology where docs are overwhelmed. I can't think of a single colleague that doesn't feel like their work is like drinking from a fire hose.

AI is great but be warry of the perverse incentive to control its abilities, just tweak the parameter to only flag things of a certain degree, ie only subpleural fibrosis of a certain amount/location and only nodes of certain size irrespective of other features. The same already happens in most emergency rooms (if they actually even have a radiologist) ie we only report that which is obviously and immediately catastrophic to life, we are not required to report incidental findings unrelated to the reason for performing the study, the indolent lymph node can wait and in any event if it does grow P will come back anyway. This is a terrible system. Same can and will happen with AI and very much so in the public health setting. Not so much a solution but certainly something to watch out for as it will fit in very nice with current issues associated with the reductionist model for medicine. Here here for a Universal Basic Income and genuinely quality health care for all - would probably change the metric across our societies - make people happier - more likely to innovate. Two cents

It will be interesting to see how onboarding AI reading of results effect error rate. Personally, I think the results should be looked at first by a human, then looked at second my AI and see if there is any discrepancies and then a second human can take a look at those discrepancies (if any)

CT scans for my chest show a groundglass density structure thought likely to be residual thymus found on abterior mediastinum. Could this be something that warrants additional tests or second opinion by different radiologist?

Its probably b3cause how competive schools are now with students having to get all As to possibly get with schools rejecting students that have a 4.0 gpa my school only accepts 30 students per year but gets between 200 to 400 applications only interview the top 30 applications based on points systems getting all As in your preq is not enough you also have to do a bunch of extra credit stuff like bonus classes just to get interviewed to possilby get in a long with hand written essay that getd scored.Its a two year progam but by the time you do your preq and get accepted if you even do its gonna take you as much time as if you just did a bachelors degree.Aslo when you get into this progams its really easy to get kicked out.
