Where was the cable loose? And did clips etc come loose or were they all in tact but it still rattled

Unfortunately, I did not see the issue cable itself before the fix. (I did inspect it, but the issue wasn't that apparent.) I had the dealer look at it. The tech described it as a piece of foam that had been worn through with use.
