Glad to see her lift her arm. Just had anatomic surgery 2 weeks ago. No pain but still a bit tender. I was told to keep the sling for 4 more weeks. I guess anatomic takes a bit longer to heal than reverse shoulder replacement. However I'm able to do some small motion exercises and shower. Still limited on my arm elevation but I stopped taking meds just 3 days after surgery. I just need to continue to be patience.

I had my right shoulder reverse replacement and was back driving a manual car after six weeks. I am now awaiting next month January 2025 when i have my left shoulder replaced and already my surgeon has said it will take longer to get back driving as this in the UK and as such we have right hand drive cars and the gear shift is on the left.

I’ll cancel surgery if I have to have the weight of that sling hanging off of my neck. I’ll accept a sling that hangs off of my shoulder. What I see is something I’ll never tolerate on my neck. Can you show the other types of slings?

i had replacement 12/6, and i agree this was the worst part. i asked my dr and he supplied a different sling that goes around the shoulder. much more comfortable

They saw off your bones and send you home same day. No concern for possible infection, no concern for pain. Tylenol and ibuprofen for sawing off your bones!! Not looking forward to my upcoming replacement.

@CarlaForth Perhaps you should get a second opinion? The usual course, is you are given a nerve block, prior to surgery, that numbs the shoulder for approximately 24-48 hrs. You’re given opiates, an NSAID, for 1-2 wks, plus you’ll be in a sling, that has an abductor pillow attached. You’re fairly immobilized for the first couple of weeks. They have you shower at home, with an anti-bacterial soap at home, and possibly, an antibacterial ointment, that you put in your nostrils, 5 days prior to surgery. They also do a surgical scrub, of the area to be incised to replace your joint. I felt no pain, until 30 hrs. After my replacement. I was awake, pain-free, and able to ride home about an hour after surgery. After the first week, I didn’t require pain medication, just an NSAID, Celebrex, for the second week. It really isn’t a painful ordeal, compared to several other surgeries, I have experienced. Good Luck, with find the right Dr.!

Just had mine done 12/16 you will be fine with oxycodone 5mg and make sure you ice every hour for couple weeks it makes all the difference in your healing...good luck

@17moonlite it is certainly possible, the actual sling part over the neck is the easiest, the only challenging part is the "around the waist" section. I usually recommend getting a good routine down and practicing BEFORE the procedure.

When I had knee replacement surgery 2 years ago, I had a real struggle with scar tissue developing after surgery. Recovering ROM was a race to stay ahead of the developing scar tissue. Is this not as much an issue with shoulder replacement? My TSR is coming up in January.

Aprox how long should I stay off work I am forklift driver and alll the steering is with my left arm. In a very busy factory. 3 tears on left rotator cuff .

I have had reverse shoulder replacement done in both of my shoulders and am 3 weeks post operative with the right should, I am in no pain, and start physio in a weeks time. I am so thankful I was able to be able to have this surgery, the pain relief is Incredible, the hardest thing is the 4 weeks in sling, but it is worth it, it does take 3 to 4 months to get your strength back but then you awre as good as new, I am 83 yrs old.

Good to hear this. I'm 72yrs and I've had left and right knees and hips done. I am expecting pain but controllable. I realize time is expected and I want to be able to go to the bathroom, shower, and change my clothing with limited help.😊

8 months ago I had a rotary cup surgery but two months ago I found out that I tore my rotary cup again the surgeon said he's not going to stitch me up again the tear was too large the surgeon want me to talk to one of his colleagues that does reverse shoulder surgery I'm 65 years old and good health I'm not in a lot of pain I have developed arthritis in my shoulder I still can't sleep on the side of my shoulder I had to sleep on my back I still work full-time but to be honest I don't know if I want another surgery I might just have to retire

JOHNNTY T. NELSON MD...HOW LONG DOES IT TAKE A SLAP TEAR TO HEAL NATURALLY? I'VE HAD PAIN IN BOTH MY SHOULDERS FROM A DEAD LIFTING ALMOST 3 YEARS AGO. THE PAIN HAS IMPROVED ESPECIALLY IN MY LEFT SHOULDER..HOWEVER IF I TRY TO BENCH PVER 225LBS..THE PAIN IN MY RIGHT SHOULDER IS JUST TOO MUCH AND I HAVE TO STOP. Doctor, I had a reverse shoulder replacement. Why is my upper arm still numb 8 weeks post operation and why so much pain? I can't pick anything up - not even a cup of coffee. I am feeling so sad and in pain. When will this stop? Regards, Kerri from Australia

I had this same surgery with a different doctor. I was told I could drive home but it was impossible to shift and steer with one hand completely numb. If you have a standard transmission, get a driver.

Dr. Nelson performed two shoulder reversals on me. The left and right shoulder in the same year just four months apart. He was awesome. He explained the process of each surgery from beginning to end. He told me what to expect from each surgery and he assured me that I could go back to playing golf. I am two years removed from both surgeries. I can throw a baseball, back to playing golf, working out, and living a pain-free life in both shoulders. I highly recommend Dr Nelson!

I didn’t have pain or swelling afterwards of my surgery 5 years ago. Sling was on 3 days. The only thing that was hard was getting dressed. I was back to normal in 45 days.

@foreveranuj with this injection there is not any considerable numbness that the patient experiences. The main goal is to deliver steroid medication which works by reducing inflammation for longer term (weeks or months) rather than just a few hours. And yes, most insurances cover cost of the injections.

Fantastic video, I am one week post op and not doing so bad. The pain was 7-8 out of 10 for a good few days but settled to 5 now. The hardest part of the this is sleeping with your sling but you have to do it to protect your shoulder. Hopefully in the next 2-3 weeks I will be in a better place.

what are the indications that tell you to do this procedure versus a subacromial/ glenohumeral injection. Also i think in that suprascapular notch, there is an associated artery/ vein no?

Great question. I use this injection whenever possible in patients where usually I would use a subacromial injection: rotator cuff tears being treated non operatively, bursitis and impingement findings on physical exam, and even rotator cuff arthropathy. There is a very high-quality level-1 study that shows these suprascapular injections out-perform subacromial injections in terms of outcomes and pain relief for patients. Sometimes I still do subacromial injections, for example if the ultrasound is tied up somewhere else OR if the patient already tried the suprascapular injection and it didn't seem to help much. Also some patients had previous subacromial injections that worked very well, so we decide to do the same injection again. For straightforward glenohumeral arthritis without a rotator cuff tear, I do not generally use these injections because I feel the main pain generator is the entire joint capsule, which is only PARTIALLY innervated by the suprascapular nerve. For those patients, I use ultrasound-guided intra-articular injections. You are correct, the suprascapular artery does pass just dorsal to the ligament, often you can see it pulsing on the ultrasound. While I do not worry about damaging it (have you ever tried to stick a needle in a very LARGE artery? Not easy, and even harder for a smaller artery like the suprascapular artery) I usually slightly draw back on the syringe before I give the medication. I have never encountered a complication with this.

Great treatment. Dr Nelson is incredibly. He is performing a bone graft on my right shoulder Monday followed by a total reverse shoulder replacement in six weeks. I am so BLESSED he is my doctor. I am recommending him to all who are tired of hurting and want to regain use of the shoulder. 😊 GOD BLESS YOU MY FRIEND

Hello, yes. Make sure probe is parallel to scapular spine, just dorsal to it. Make sure depth is set to 4 or 4.5, even deeper depending on the patient. If you are not seeing any bone signal, likely the depth isn't set deep enough. With probe parallel to the spine, scan laterally and you will se a "ski slope" dip/depression in the bone signal: this is the notch. If you go too far lateral, you will hit acromion process. Too far medial, no ski slope.

Hey, I had subpectoral biceps tenodesis just under 8 weeks ago. I have a few questions, if you don't mind answering? 1) How strong is the anchor in terms of lbs/kgs pull out strength? 2) If I were to supinate my forearm with no resistance/no weight on it and my bicep tenses, would this affect the repair? 3) Can a bicep tendon slip out from under the anchor over the recovery period if the bicep is tensed?

Great questions. In terms of pullout strength of the anchor or fixation device in kg, that really depends on the particular device. For my subpectoral tenodesis patients, I use a device different from what is in this video called a cortical endobutton which has an in-lab pullout strength of approximately 25kg. This doesn't sound like much, but is certainly more than enough for the forces exerted through the long head of the biceps. To answer your second question, a well-placed subpectoral biceps tenodesis can certainly withstand simple active forearm rotation without fear of rupture. While you need to consult your surgeon about your case, in my patients I allow them to move their elbow and rotate their forearm as tolerated for "activities of daily living" immediately after surgery, and at 6 weeks after surgery they can begin some gentle, low-resistance and low-frequency strengthening of the biceps and gradually increase resistance over the following month.

Great Video !!!! As a Surgeon do you prefer the high in the groove Tenodesis or Low in the groove. Tenodesis for your patients??? Once that tendon is fully healed into the bone is that where the blood supply now feeds the Long Head tendon ??

At around or just under 6 months from surgery, if a therapist was to do deep tissue massage into the site where a subpectoral biceps tenodesis is located I.e. pressing their fingers deep into the insertion point where the tendon and anchor are located. Would or could this deep massage and pressing deep with fingers damage the tendon, or damage/compromise the repair itself? I understand that my surgeon used an Arthrex Fibertak biceps all suture anchor.
