Thank you for making these videos public. The cases are great, and I have learned a lot of body radiology from these videos. One tiny suggestion. It is sometimes a little difficult to understand Dr. Victoria's cases. I think her microphone may be having some issues. Her cases are amazing, and I would love to learn from her.

There are several molecular types of hepatic adenoma. Imaging may corrleate with molecular subtypes but overlap 1. Inflammatory subtype is the most common type of hepatic adenoma, which shows T2 hyperintense, hypervascular and more likely to bleed 2. HNF-1alpha inactivated(mutated) subtype usually contains fat in itself and shows moderate hypervascular pattern, no bleeding, relative homogenous texture and often multiple. 3. Beta-catenin activated hepatic adenoma is classified into 2 subtypes which is CTNNB1 muatated in exon 3 and exon 7 & 8. Adenoma which has CTNNB1 mutated in exon 3 usually occurs in androgen therapy, liver vascular disease and is associtated with HCC transformation, colorectal cancer and medulloblastoma. But adenoma which has CTNNB1 mutated in exon 7 & 8 has low risk of malignant transformation This subtype is known to show hepatobiliary phase hyperintensity which is distinguished with other types of adenoma. but inflammatory subtype can show HBP hyperintensity. 4. Sonic hedgehog subtype is less common and known to have high risk of bleeding. 5. Mixed or unclassifed When you detect adenoma with large(>5cm) and heterogenous texture, you should be aware of the risk of malignant transformation of it

What is the probability of a complication resulting in the complete REMOVAL (not just a cut) of the CBD, the hepatic ducts, the cystic duct, the duct of lushka and severing of the right hepatic artery and causing damage to the liver bed during a lap chole, plus the surgeon not spotting it in either the original operation or a follow up laparoscopic investigation 27 hrs later?

Adenomyoma will have glands and some amount of smooth muscle around glands while endometrioma will have only glands, pathologically.

38 m Canada here. I have a congenital absence of my IVC. All of the blood from my lower body returns to my heart through my azygous/SVC. It was first diagnosed in 2002 but the doctor had never seen the condition before so there was never any follow up. In 2017 I developed a blood clot from the top of my right knee to my pelvis. After a round of Tinzaparin injections I was put on Eliquis. While on the 2.5mg dosage I had another blood clot in 2018 and another suspected blood clot in 2019. Since I kept clotting a hematologist put me on Eliquis 5mg twice a day indefinitely and I haven’t had any issues since. Whenever you see somebody with my condition please monitor them closely for DVTs and consider anticoagulants as a preventative measure. My superficial femoral vein has either completely clotted or collapsed and I have chronic clotting in my common femoral and iliac veins. My right leg causes me pain every single day and I live in constant fear of what the next blood clot might do to me. If the first doctor I saw suggested any monitoring or preventative measures it may have significantly improved my quality of life today. Good luck to this patient.
