Usually when we do an access through the crown, there will be no damage to the crown. But this crown had a very thin layer of porcelain,so it chipped off like that. Usually an access through the crown will only have a small circular opening.

I know there are many philosophies but I apply the preventive measure of opening the pits and fissures , it's not a critical part of the tooth and I will be preventing microbes entry, especially with all the sticky foods that we have nowadays. So on the long run I will be more conservative than your approach

Heyyyy... That's a good question, it's not a must. I removed all the pulp and debris from the canal and immediately obturated. Caoh2 is a good medicament, but in some cases, I choose not to use them. It not 100% necessary. 😊😊😊

I don't recommend bonding resin right over an exposed pulp. Usually you would disinfect the prep with bleach and then cap the pulp with MTA or Calcium hydroxide.

Heyyyy.. I totally agree with your opinion. I would do the same thing if that pulp was actively bleeding. In my humble opinion, the pulp has regenerated, that's why there Is no active bleeding. I removed the caries around the exposure space and placed theracal over it. This has been my protocol for half a decade and surprisingly it works. I follow sodium hypochlorite and mta for actively bleeding pulp. As I said, just my humble opinion 🙏😊

The files I'm using are from eighteeth, e-flex blue. Size that I'm using is T1 : 20.04(yellow) T1.5 : 25.04 ( red) T3 : 35.04 ( green)

Can you do a video showing ergonomics and how you can fit both the mirror and file in the tooth and placing files in? I really struggle with seeing and also placing files in the mesial canals due to the angle and running into the dam on the opposing jaw

I use a tweezer to hold the files and place them in, so this enables me to not put my fingers in. Mirror size, I'm using a size 5. I usually place my mirror slightly away from the working tooth and use my microscope to focus on the mirror. It's abit hard to explain, but try to imagine that I'll place my mirror at the premolar region to look at the 6, sometimes at the 7 to look at 6, never at 6 itself.

Bro, can you suggest a long shank bur for air rotor, with perfect length and size for good access visibility during access cavity preparation, because I can see that the bur you are using has sufficient height and size and still the access is completely visible.

I would love to suggest some to you, but different suppliers carry different burs and all of them have their own way of numbering them. A #3 from mani might be equivalent to a #6 on Mr. Bur. So, I can't specifically suggest a bur to you. But what I would suggest is asking your bur supplier for a long shank ( abt 7mm) diamond round bur( 2mm-3mm). This is perfect. Anything bigger will be too much .

Hahahahhaa... The thing is, I'm doing these cases clinically and usually a large mirror doesn't have similar capabilities as my small mouth mirror in reflecting light into the canals when using a microscope. That is why I stick to my mouth mirrors although sometimes it's hard to angulate. And also, it's a 2nd molar and a real life patient. Sometimes I get lost in the focus of treating the patient rather than recording the case. 🙏🙏🙏 Sorry. I'll try to take better videos next time 😊❤️ thank you for the support

Just a gut feeling that it can't be this simple. Since I have a cbct at my disposal, I took it. But if I didn't, probably would have obturated that two canals and moved on 🙏😊

Once again, amazing and informative video doctor thank you . M was light contact with the upper premolar. Is this something that would concern you going forwards, i understand an indirect restoration would allay this concern, but if the patient would not want one for a while would you be happy to leave the contact on the m aspect as it is ?

Heyyy.. Yess.. I use alcohol and cotton pellet. Also, scaling helps. After 90% removal, use etchant and scrub with a micro brush.

But, this tooth has got Alot of structure left, in my humble opinion, a composite veneer will do the trick. But ideally, crown would be a great option too

Yes it is. But I would advice trying to do crown down technique if you are using rotary. Don't try to bring your rotary straight to WL without using a K file first. I don't use barbed broach in my daily Dentistry too

Do you mean the one I'm using to carve? It's just a diamond shaped sharp composite carver from a local brand called quensdent. It's something similar to lm arte's fissura. 😊❤️

Always used side vented. It's the safest way to prevent over extrusion of sodium hypochlorite beyond the apex 😁

I prefer using ribbond, as, if it fails, it won't cause a catastrophic failure unlike a post which will fracture the root of the tooth. Ribbond functions similar to a post too 😁

Heyyy. The purple tip is actually the irrigation. I'm using full strength sodium hypochlorite. The green tip is my sonic activator. A sonic activator activates the sodium hypochlorite irrigant which will help it flow into any accesory / lateral canals if present 😁❤️

Nice video doc...😊 Small query....in a case where DME ,DMA ,to be done very deep cavity after struggling with isolation,teflon packing, and raising the box upto 2mm so make it Supra gingival...while placing new matrices if contamination occurs of cured composite.... what should be done.

You can definitely etch and bond again if contamination happens. But if it's just a little blood seeping in but, u can try wicking it away with a micro brush. But I suggest reetch and bond. 😁

Heyyyy dr. I don't use any liner. I'm just using flowable composite at the deepest end. That's why you see me curing. If you are using gic, I would recommend you to place the gic as your base first, then etch and bond the tooth as well as the gic and proceed with the restoration. Hope this helps. But I personally do not use neither gic or any liner. Thank you for the support 🙏🙏❤️❤️

This tooth in the last tooth in that quadrant. So we are forced to place the clamp on the same tooth we are planning to restore. It's always a good idea to place the clamp slightly more subgingivally to have space for clamp position. Appreciate the kind comments 🙏🙏😁😁

Not really anything in particular, I think they all work fine 🙏😁 Teflon is magic, takes time to get used to it, but once it's incorporated into your workflow, you can't live without it

Cool technique, can be useful for class II where there’s gag between the matrix and box. For this case, why not just trim and finish directly since there’s no adjacent tooth?

You can always do free hand technique for this case, but you'll never get a well adapted margin, it'll always be an overhanging one. Using matrix will always be the best choice 🙏🙏😁😁

Yes.. It's just a white stone primarily used for polishing, but I use them to remove unsupported enamel 😁❤️
