There's a mistake on the slide of when to defer ART, the 2023 guidelines says that in a patient with a positive serum Crag and a negative LP, that ART can be started immediately where as the slide says defer for 2 weeks Also the Clinical 2023 guidelines differ from the SA HIV clinician society guidelines regarding VL monitoring. The Clinical guidelines mention the VL at 10 months after starting but the HIV Clinician Society doesn't - it mentions doing a VL at 3 months then if under 50 a repeat 3 months later (6 Months) and if under 50 then 6 months later (1 year) and if there is ever a VL above 50 one is to intervene and repeat in 3 months time Can you please advise on which to follow regarding practice and for the HIV Diploma exam

Thank you! There were at least three versions of the ART guidelines released last year and this presentation was based on one of the early ones. There were two small changes in the later release (version 4) - one is the recommendation that ART can be started on the same day in a patient with CrAg positive and a negative LP. The other change was an update on the TPT for children, with the recommendation that all children over 14 weeks initiated onto ART also receive IPT cover. Since the NADIA trial. Do we now switch patients on 2nd line AZT + 3TC + Lpv/r to TLD despite there being resistance to both NRTIs’s. Assuming patients are virally suppressed. Hence we can recycle the tenofovir?

That is correct - TDF is being recycled as in trials it performed better than AZT in combination with DTG as a second line regimen.
