Thanks for the video! Lots of great info on nicotine. I would like to note that while the total nicotine content of a cigarette is 10-14 mg/cig, most of it is not inhaled or absorbed. It is more along the lines of 1.2-2 mg per cigarette that is inhaled and makes it to systemic circulation, so the total content of the pouch is more in that sense, but considering the first pass metabolism through buccal absorption, I think the 3 mg pouch will deliver the total systemic nicotine content of a typical cigarette.

Thank you for researching this topic. The impact of this Nitro phobia potentially prevented treatment to AMIs. The review of the data is greatly appreciated. Have you considered the topic of adenosine for SVT due to WPW. Here in Central Indiana some medical directors are stating WPW is a contraindication for adenosine.
@jamesdesrochers4383 ай бұрын
One of the big reasons is because WPW is an AVRT not AVNRT, by blocking the AV node all your doing is sending conduction down an unregulated accessory pathway which can get bad if the pt has multiple accessory tracts. Still may be safely effective in some narrow complex but still gotta be careful and make sure theres not an underlying afib, have you been successful with adenosine/wpw svt in the past?
@wendellhoward8843 ай бұрын
@@jamesdesrochers438 Thank you for your reply and contrasting AVRT vs AVNRT. Yes I have been successful with SVT and history of WPW. But I am cautious about drawing "facts" from my practice solely on my paramedic anecdotal experience. And I teach my paramedic students about the dangers of A-Fib RVR and history of WPW. When I ask medical directors about WPW as a contraindication in the protocols, I finally received a legitimate answer from a respected ED physician. I introduced my questions with the FDA and drug manufacturer's approval of adenosine for WPW SVTs. She honestly responded, "that is how I was taught in medical school." I try not to teach dogma and share both sides of the opinion, and unfortunately many times I am warning the students about how anecdotal information is becoming "fact." Any more insights you have to share I pass on to the new generation of paramedic students.

I quit smoking (44 years) by using the pouch. I knew there would be issues with the other aspect of smoking unknown triggers, hand to mouth habits, planning to not run out of cigarettes, drinking, and social cues. So, I had two pouches and bought hemp cigarettes. Instantly, I have not had a bit of interest in smoking a cigarette 0! Because the hemp cigarettes tend to be harsh, I very seldom feel the need to smoke them and i smoke them like one would smoke a cigar. After three months my lungs are clear I don't stink, my house and car doesn't stink. I started with 6 or 8 mg and have been tapering down to 3 and 4mg doses. I cannot believe in two hours i completely banished cigarettes from my life. I had tried everything and this is working. I hope others get this message.

Additionally, we already have trials showing the risk for aspiration is low in NIPPV patients which is why NIPPV on restrained patients was changed from an absolute to a relative contraindication.

Maybe it’s just me, but I feel like this is one of those studies that falls into the category of common sense and just proves the ignorance of the medical field. We have somehow attached ourselves to this notion of “it there’s no study to back it, then it must not be good!” I don’t need a study to tell me that an ultrasound Doppler is more sensitive and specific for monitoring the femoral pulse during a code when compared to manual palpation, but they studied it. I also don’t need a study that tells me NIPPV prior to intubation is superior. This feels like common sense.
@rubenrobledo7665Ай бұрын
Thank you! I thought I was the only one thinking this. We’ve been doing this prehospital setting for years, if there’s no bipap we have bvm O2 with peep set 5-10 at least 3min prior to intubation. But it takes a study to confirm even the common sense practices

Aren’t insecticides harmful to humans in general? Wouldn’t nicotine be safer to use in agriculture?
@Brandon-yq1tm4 ай бұрын
Pretty much, but you can't really put a trademark on something on nicotine and control the sale of it such as you can on Monsanto's Round Up, which does cause cancer. But who cares about that as long as you are making a profit.
@gwhizz89223 ай бұрын
Yes, nicotine is safer than modern insecticides. In fact, most vegetables have nicotine in them. Eggplant veggies have the highest content. It is a natural protection for the plants from insects although some insects are immune to the nicotine.

Keep in mind that the EPA is a tool to create policies, pushed by lobbyists, onto politicians, that will create rules or boundaries that will benefit their industry. That’s usually by banning or limiting another industry for something that’s not real.

They come out with nicotine pouches...nicotine is pretty safe except it causes horrible gum recession. They decided to use ACESULFAME Potassium as a sweetener ...it hasn't been tested enough to be considered "safe." Can enhance tumor growth and stomach issues (heartburn). These pouches would be safe if they didn't dump ACESULFAME into them! It's probably way worse than nicotine or tobacco. They get you coming ..and going. They need to add crap that'll kill you or cause tooth loss due to holding nicotine against your gums for hours.
