VSD) with atrioventricular discordance are quite rare. AVSDs themselves account for about 4-5% of all congenital heart defects1. When combined with atrioventricular discordance, the incidence is even lower. For instance, congenitally corrected transposition of the great arteries (L-TGA), which involves both atrioventricular and ventriculoarterial discordance, accounts for less than 1% of congenital heart defects. In case of complete AV septal defect distance from apex to mitral valve and tricuspid is equal. In that case identify the intra-ventricular morphology, you have to find out intra ventricular structure in case of right ventricle. Moderator band is structure seen exclusively in right ventricle. right ventricle surface rough compared to left ventricle. In this case, the left ventricle is connected to left atrium because that ventricle does not have any moderator band like structure. Incidence wise relation aorta to pulmonary artery should be right sided. that is the scenario when both ventricles are normally related to each other. if right ventricle and left ventricle not normally related then this is possible. in this case right ventricle relatively more anterior to left ventricle, than common scenario of ventricular relationship.

Thanks but first how D-TGA and aorta is anterior and to the left ??? It should be anterior and to the right Second how did you know that this is tricuspid atresia not mitral atresia especially aorta is to the left of PA which goes with L-TGA and L-loop of ventricles

We usually have confusion at that level, but after careful observation, narrowing of pulmonary artery is not significant one at the level of tip of the pulmonary valve. Thanks

@@NouranMohammed-pj5jb Biventricular repair is a good option for patients with an unbalanced AVSD, which is when one of the two ventricles is disproportionately small. Most patients who are eligible for biventricular repair have the procedure before they are six months old. The mortality rate for biventricular repair in symptomatic patients who are three to four months old or younger is less than 3%.

Atrioventricular septal defect with balanced ventricles A type of atrioventricular septal defect that can include a primum atrial septal defect, a left-sided tricuspid valve, and a deviated atrial septum. Repair options include closing the septal commissure, removing the lower part of the atrial septum, and inserting a pericardial patch.
