I know Successful Failure was formed know as Broken Thumb in 1996. Broken Thumb recorded only song called No Reason (re-titled as Streets in Avenged Sevenfold - Sounding The Seventh Trumpet). However changed their name as Successful Failure same year and recorded 6 songs between '96 and '97.
