Sadly, Dara -- maybe for comedic effect, or maybe out of obeisance to medical science, or maybe out of outdated knowledge -- distorts the actual circumstantial FACTS about cuts or tears. If a vagina seems too small for a baby's head, and has prolonged labour beyond what an obstetrician considers a "safe time" they often make an episiotomy (longitudinal cut) all the way from the perineum to the anus, thereby, obviously making, yes, an easier, larger opening for delivery. This cut may be up to 3"long as they often like to insert forcepts AS WELL. However, if not rushed out of fear and cut prematurely, or cut for delivery "convenience", or massaged to induce greater stretching, the vagina may only tear 1/2 to 1" to allow the baby's head to pass. A tear never extends to the length of a cut. So take your pick: a longer, possibly unneeded cut, or a shorter tear only as much as the delivery actually requires. As such, episiotomies have actually fallen out of favour. "Experts once believed an incision would heal better than a natural tear. The procedure was also thought to help preserve the muscles and connective tissue that support the pelvic floor. Today, however, research suggests that routine episiotomies don't prevent these problems after all". Mayo Clinic. A geat piece of storytelling, though.😅

Some 'fun' context for the doctor midwife war: one of the earliest cases that proved germ theory came from a hospital with two maternity wards, one run by midwives and one by doctors. A man called Ignaz Semmelweis came in and observed that the doctors' ward had significantly higher death rates for both mother and child, and eventually (correctly) concluded that it was because the doctors were performing autopsies, with their bare hands, then going straight into examining laboring mothers, with their bare hands. So corpse bacteria was being introduced into the uterus, as well as into all the tears and cuts that come with labour. The mother would then contract puerperal fever and die. Semmelweis got the hospital to trial a standardised hand washing procedure for a few months, during which the death rate dropped EIGHTY PERCENT. However the doctors didn't like the notion that they were responsible for killing their patients, so they renoved all hand washing procedures and had Semmelweis committed to an asylum (which, to be fair, he was spiraling due to his depression over not being able to save people when he clearly knew how to, started drinking tons and having sex with a lot of prostitutes, so the asylum treatment wasn't entirely unfounded). Semmelweis was then beaten on arrival to the asylum, and died two weeks later from an infected wound on his hand, in a tragic twist of irony. Louis Pasteur then more formally established germ theory years later. So, while modern doctors are obviously much safer and oftentimes a necessary precaution in a birth with any chance of complications (aka any birth), I think the midwife doctor rivalry is too deeply rooted to be overcome in just a few decades of doctors being kinda reliable at their jobs, given historically midwives have always been the ones with a better grasp of how to do their jobs safely.

It's about informing ourselves about things well enough that we don't fall for all the propaganda, psuedoscience, misinformation and disinformation out there. By "informing ourselves", I'm just talking about taking the time and effort to really learn how/why science works, why it's so crucial for getting at what's true (or at least what's most likely to be true). On top of learning about the methods of science and why they're essential for truth, it's important to learn about the numerous cognitive biases, as well as flaws in our memory, attention, vision, etc. The journey through all these various concepts can have a profound humbling and freeing effect. I also tend to recommend for people to learn about the history of Conspiracy Theories, and how their psychological traps keep us from thinking critically about things. It took me years to gain enough "intellectual tools", to finally pull myself out of the downward spiral of thinking (such as conspiracy theories). I truly hope you can find your way through all the BS out there, I know from experience how hard it can be. If your curious, I would be more than happy to send you some videos on the basics of the things I mentioned. If not, I truly wish you the best of luck on your journey man 😌

While I absolutely love Dara, and am 100% with him in denouncing pseudoscience like homeopathy and the like, in this particular case he is making a mistake, albeit an understandable one. While it's hard to believe that a 'tear' can be better than a surgical 'cut' in this case it is true. The procedure is called an episiotomy and there is a solid chunk of research backing up the finding that a natural tear healed better and with fewer long term consequences. This overturned years of medical practice during which this procedure was considered routine and performed in the majority of births. Recently the guidelines have been revised, recommending an episiotomy only in specific circumstances. Just to be clear I in no way support these midwives and shamans or whoever that demonize doctors and medical assistance during the birthing process, and who inflict tremendous guilt on women who seek medical assistance in dealing with labor pains. It just so happens that in this case, experimental evidence happened to agree with them, and I bow to the evidence.

A recent Cochrane systematic review examining the evidence on selective versus routine episiotomies for vaginal birth concluded: Overall, the findings show that selective use of episiotomy in women (where a normal delivery without forceps is anticipated) means that fewer women have severe perineal trauma. Thus the rationale for conducting routine episiotomies to prevent severe perineal trauma is not justified by current evidence, and we could not identify any benefits of routine episiotomy for the baby or the mother.” A mediolateral incision should be used i cases such as - breach delivery - foetal distress - instrumental deliveries A cut always heals better than a tear. The question is whether that tear would have occurred in the first place, and how bad would that tear have been. Routine episiotomies assume that you would have a severe tear without them, which we now know is not true. A selective episiotomy, used in the right situation, can save you from a 4th degree tear.

As someone with female-assigned genitals and chronic vulvovaginal pain, I know firsthand how little Western medicine understands vaginas. Up to 28% of women in post-industrial countries will have vestibulodynia at some point in their lives. It is basically medically unexplainable pain in the vulvar vestibule (area around the vaginal opening). In seeking treatment here in the Midwestern US, I experienced misdiagnosis, being prescribed antibiotics and other drugs I didn't need, sexual harassment, and general disrespect and medical neglect. Doctors don't understand vaginas. It is a big problem.
