This is not a good example. If you don't know the environment and the rules of it, it's ok to behave like a sheep. It's worse in situations when people have enough information to figure stuff on their own, but they follow the crowd.

Excellent presentation and slides, though somebody really should have coached the facilitator around microphone placing, which was incredibly distracting and made it difficult to listen to.

Did you consider stroke repats, MH patients, ACU , clinics, emergency crash calls, electives all this will affect beds and ED flows too

Being a victim of D2A and having read the guidance. 2014 Care Act. Connected Lives , Etc etc. It's all nonsense. It seems the ones who should be Caring spend too much time writing manuals and making videos. I was discharged from hospital after domestic abuse to a BUPA care home for six weeks aged 60. Im still here 19 months later. ! It's making me ill.

The PDSA cycle was actually famously created by W E Deming who is not credited. Langley Nolan and Nolan only added the top bit. (Which is arguably just another PDSA cycle)

Good overall. But there is confusion between whether Nick is talking about ordered systems (where the mechanical, factory methods hold true) and disordered systems which are non-linear where improvements are not consistent predictable upwards trajectories. (Where the gardening metaphor holds better) Also a sidenote. You can't solve wicked problems by getting to root causes. That's what makes the problems 'wicked'.

You are not the Dr. Fred Bell I knew. You are much younger. You probably did not work for NASA & teach us about his experience with the Pledians. A simple name. You are not who I was looking for. He had a ratio show. He had an 😍 bg tape series on who he learned about the Pledians. I am sure your work is important too. We do not know what kind of Dr you are.

Really interesting, thank you for posting. Can you please point me to information about 'Criteria to Reside' and how this differs from 'Medically Fit for Discharge'. Not sure I've heard this used in NHS Scotland (but I'm not frontline, so I could be wrong). Cheers. Alan.

I know this was an artificial situation but I felt the lady (consultant) did not great and explain what she is doing to the patient. It looks a bit unprofessional even if it is not a real situation. yet it is very useful to juniors
