Cracks are ubiquitous most of them go unoticed, i am pretty sure that if we had a means to detect every single one we would be shocked. Stance should be to notify the pt, drilling asymptomatic non decayed cracks out as sugested by many seems a little invasive to me, remember primum non nocere.

I would love to hear your opinion on the reciprocating files. I’m only a year out and don’t have a ton of experience, but personally I have found that I like wave one reciprocating files as my second or third file, but the initial glide path is much easier establish with a traditional continuously rotating file. I’ve used wave one glide path file and it constantly gets stuck half way down the canal for me and I feel has a tendency to push debris apically. Whereas I’ve tried a pro glide file for my inital glide path which is continuously rotating and it is always able to make it all the way to the apex no problem for me and doesn’t get jammed up like the wave in glide path file does.

Its a single file taper , so taper is too large for small width canals . Every cade is different , does this taper manages every kind of canals ?

Ash - two things - why the fiber post? What core are you trying to retain there? You're already bonding to the walls, there. Second, since you didn't cover the post with composite to seal, aren't you going to have wicking of saliva down into your pulp chamber? Did your root canal course back when I was in the CAF. Good stuff, that's why my questions above.

As someone who suffered day and night for weeks with a tooth abscess back in 2019, this is one of the 10 worst physical pains to experience, it didn't take until speaking face to face with my dad for me to go see a dentist to remove the ball of fluid from my face. I wish no pain on anybody especially this. All the slobbering the tossing and turning the whining complaining the gnawing holding the side of my face glad that's over with

I took a recent ce course that stated 3% of all cracks result in tooth loss. I would inform the patient and leave it alone. If the amalgam needs to be replaced I would explore that crack and consider a class ii if there is still sufficient cuspal integrity

That’s how my dentist fixed my chipped veneer. He put the wax on the missing part and then took an impression and made me a little crown. Hopefully my new zirconia crown will be good !

Big fan of the blackjack file from planb dental (minikut). It’s great for that coronal access. Easier than using the wave 1 to open up the canals.

I just had my second molar removed it about 6 days ago and it still really hurts the dentist did it because of and infection root canal tooth it took and hour and a lot of pain to extract it I wish she had better informed me of options and told me it was fused to the bone a bit i would have opted for surgical removal under anesthesia instead of this along with stitches a open hole in the back of my mouth just killing me and having to take 1500 mg of Tylenol is not killing the pain I’m over this

I wish all Endontic were as careful as you. I had two separate molar root canals and both edonitic missed the fourth root. Now both teeth are crowned. Does the root have to be found and removed?

I like to use the existing cusp and anatomy as reference to locate my orifices. And then I do occlusion reduction and finish the endo.

My experience with crack lines in amalgam filled symptomatic teeth regardless of the filling size is negative (may even be a tiny occlusal). RCT and/or crowning such teeth is unpredictable - we may slow the crack development, but no promises. I agree extraction should be discussed with the patient as the inevitable option, emphasizing that even crowning is not sufficiently safe. I wonder if transillumination is helpful with more accurate diagnosis and prognosis of crack lines in general?
