@@maryalgar8779 absolutely! You can definitely decrease the pain, strengthen the surrounding muscles, and have no issues. You may have a predisposition for it coming back, in which case you use similar strategies for decreasing it before it becomes a big problem again.

@maryalgar8779 yes, definitely! If the OI muscle is tight, then it can create less space for the obturator nerve. Flossing the nerve can help it improve mobility and be less "sticky" in spots. This is a great complimentary treatment to improving mobility and strength in the obturator muscles 🙂

@@margoochoa2419 doing the figure four stretch or pulling your knee to opposite shoulder would be nice stretches for the OI.

The OI can spasm and clamp the pudendal nerve, causing a neuralgia. Releasing the muscle to free up the nerve is so simple conceptually, yet so challenging…

@@beckybauman-bc9xb potentially, if you have your hips positioned where the muscle is on stretch (knees inward), or if you are really shortening the muscle fibers with external rotation (knees out side). But that is not a typical pain presentation. If you are having pain with sitting, there is probably something else going on that needs addressing!

When I do this, I do the fog a glass breath trick. My ribs go down first but it feels like it helps me find the lower abs. Also feels like it opens up the back and side of my ribs. How does all of that sound? It looks different from me gripping and feels easier to breathe. Just wanna make sure if my ribs go down first does that mean I am still upper ab gripping?

Good question! Breathing like you are fogging a mirror works really well for a lot of people to find their lower abs. Because it is more driven from your breath, it makes sense that it starts at your ribs (where your diaphragm is located). As long as it feels like everything is lightly activating, I see no reason for that to be an issue, as long as this still works for you when you are doing stabilization exercises or need stability for functional tasks (i.e. picking up something heavy).

You need to keep making videos lol. You talk about a lot of things that inter loop with males who are battling pelvic floor dysfunction. I have had it for 3 years and a lot of your videos are about things I had to learn on my own. I am in no ways or means trying to tell you what to do with your videos but for some reason the way you record your videos and the way you describe things is really easy to digest. I request one knee pain video. I have swayback, which may not be a huge problem for many but I believe my adhd brain made compensations for it that negatively affected how my pelvic floor works for me. My muscle isses: Weak ankles, tight bottom feet, knee pain, tight glutes, weak or tight hip flexors and core dysfunction with a tight mid back. Just in case you need new video ideas lol. I laughed when I saw the first comment was from a fellow male pelvic floor dysfunction patient. Regardless just wanted to tell you how good you are at describing and explaining things. Keep doing them! *somehow paragraphs got switched around in my comment! 😂

Thank you for the nice feedback! It is always nice to hear, because making these videos definitely puts me well outside of my comfort zone, and can be very challenging. I do have a running list of other videos that I want to make, but have been busy with full-time work, plus my own practice on the side 😬 . The other challenging part is making a video that is helpful for the general public, and not too specific, because not every exercise is appropriate for every body part pain. I have seen a lot of people in my office that tried to DIY their own PT at home, and ended up making their pains worse. I will certainly add your thoughts to the list, though!

What do you think about pelvic wand for puborectalis muscle dysfunction?

It depends on what type of dysfunction you are trying to treat. If it is hyperactive, the pelvic wand can be great for reducing that, which could reduce tension on the tailbone. And the pelvic floor can definitely be overactive if there is acute hip pain sending the body into a guarding response. If there is puborectalis dyssynergia that is affecting bowel function, I would want to find the pattern of dysfunction first before recommending a treatment. The pelvic wand may be helpful for overactivity, but not necessarily for coordination issues.

Thank you. Yes, it's dyssynergia. My pelvic floor specialist recommended TENS 7000, because of the affordability.

Interesting! My hip doctor identified the OI is my issue, but my pain is all in my sit bone and my pelvic floor (where the OI attaches to the pelvic bone). Thought it was a proximal hamstring tendon injury for a while because of my sit bone pain. But you didn't mention that area as a common pain site. Am I an unusual case?

No, I wouldn't say that is so unusual. Because the OI shares connective tissue with the pelvic floor muscles, having pain there is not atypical. The proximal hamstring sensation doesn't surprise me either, because they are so close together, and are influenced by similar hip and pelvic movements, it seems natural that your brain perceived it as your hamstring.

@@oxbowhealthwellness Oh okay, good to know. I've also had pain going down my hamstring, and I apparently also have pudendal neuralgia too. Just a lot going on in there. ☹️

​@@oxbowhealthwellnessanother question - I tried doing this, today. After watching a couple similar videos. And I just had a lot of that nerve pain afterwards. Does that mean I shouldn't be doing this? My doctor also said my obterator internus Tendon is thickened. I start pelvic floor PT in two more weeks so I was looking online for things to do in the meantime.

@@ginnytilby oof, yes, I would hold off. There's a good chance you could be irritating the pudendal or obturator nerve, or maybe just pushing too hard. I have some other exercises that could be more appropriate, but also more specific. I love that you are proactive, but maybe wait until you have a full assessment and specific treatment plan from your pelvic PT ☺️

Whoa! My pain is excruciating in the pubic synthesis. I cannot walk around the block. Or get out of my car, or stand without stabbing pain. Also deep glutes. So exhausted. This has been going on for 4 months!

Sometimes the spasmodic OI refers pain to the pubic symphysis, but intense pubic symphysis pain for a prolonged period of time is usually caused by other pelvic asymmetries. I recommend seeing a pelvic therapist. Or, we can do a telehealth consultation if that would be helpful 🙂

I’m always tight in the hip flexor, IT band, and hamstring groups. It does help quite a bit to do some ROM wamups before activities.

Yes! It also helps a ton to do light hip flexor strengthening, and lateral hip (glute medius) strengthening. Especially if your IT band bugs you a lot.

@@oxbowhealthwellness My IT band(s) don’t bother me at all, but both sides are always crazy tight. Like they are the suspension cables holding my bridge together 🤣🤣. Hamstrings are the same. I have to do some warmups for them to relax enough to do the palm on floor thing.

After some of that hands and knees as warmup I like to do open books and supine trunk rotations, I get huge spine mobilization benefits from those.
