Hello, My med school was extended by 1 year due to Covid 19 I gave all exams on time so my transcript has same dates as they should be but my clinical rotations that I missed during Covid were conducted later so the med school duration is extended. This is for all the students of the school. What should I do?

We are strong fans of Pass/Fail for Step 1, regardless if IMG or USMG, since we compile ERAS packages holistically and do not just focus on the USMLEs. As a matter of fact, passing USMLEs on 1st attempt is more important than score, according to NRMP's 2021 PD Survey. With all of this said, we at ACMedical are more concerned about the rising importance of CK score for a minority of PDs, since there are no other numerical values they can rely on to replace having to look at ERAS applications holistically. We're waiting and watching for the 2022 PD Survey from NRMP to provide more clarity on this from actual PD responses.

Thank you, and its our pleasure! Two answers: 1. Legally: It depends on which agencies it was reported to. I am not sure if there is any reciprocity between the medical board in that country, and the U.S. You'll need to seek guidance from an attorney for guidance here; let us know if you need a referral. 2. Ethically: Yes you need to mention it and be able to discuss it, however before you do you should speak with a strong mentor with experience in how such disclosures may affect your ERAS, and to prep you on best way to present it very similar to an interview prep. Below are my responses: By AnnaLisa Wilson: "How are PDs reviewing applications" @AnnaLisa Wilson: This year it will be very very different, due to COVID. We just finished a webinar on "Preparing for Residency Interviews During COVID (Part 1 of 2)" which we will post here by Oct 28, 2020, so please watch it for a very detailed look at your question (and be sure to sign up for part 2 on acmedical.org/residency-prep). In summary, they will: - Look for the value you will bring, your resilience, and adaptability, all fueled by COVID: “What did you do when the pandemic started?” - Interviews will be online - They will use social media to showcase their own strengths but to also look for you - Use screening tools like supplemental applications, and online skills assessments By Manish Shrestha: "When do you typically start getting interviews?" @Manish Shrestha: As of now, Oct 25, 2020, our member have not reported any interview yet; programs have been sending secondary applications and requesting for online skills assessments; we expect first interviews to be released by Oct 30, 2020. Hello @ Dr. Mondal. Great job on your USMLEs, however as you know, USMLE Step 1 score is already set to go away, and Step 2 CK score will be next. So you should focus on all the attributes that PDs look for outside of USMLE scores, which you can find in the 2020 NRMP PD Survey, such as LORs from specialty, quality of your MSPE and your commitment to these specialties. Also with super competitive specialties like the one's you're looking at, you'll need to have a solid US clinical experience in those specialties, but more importantly focus on just 1 specialty. Also note that cardiology is a fellowship, which you'll apply to after IM residency. Let me know of any follow up questions.

Assuming you're an IMG, then I suggest only 1 specialty (unless applying to Advanced/PGY2 specialties). With that said, 6 interviews in the same specialty is the magic number for IMGs for IM & FM; for most competitive specialties (meaning with higher # of US medical grads) the number should be 12 or higher. For USMGs the magic number is double (since they secure so many more interviews). Also the more red flags, the more programs one should apply to; say it took 2 attempts to pass a USMLE, then that individual needs to almost double the number of same specialty programs they apply to, and that is with a well put together ERAS package (4 recent LORs, continuous US experience, 2016/2021 compliant MSPE, applied on ERAS season opening day, etc.). These are how you get to the magic number for the specialty you're applying to. See Charting Matches Outcomes > # of contiguous ranks that led to a Matched for each specialty. Hope this is helpful.

Why do some people say to waive your right to see your LOR, and some say do not?

Here are some reasons why they recommend for you TO waive: - They truly believe that waiving your rights keeps the process of recommendation unbiased from your opinion - They don't want you to go back for edits - They don't want anyone to have copies of their personally written LORs - They want to copy/paste from previous LORs they've written Reasons why some recommend that you DO NOT waive your rights: - They want to protect you - They've seen how poorly most LORs are written - They know most LORs are generic and want you to professionally push the writers to step it up and make it more personalized - They know most people who say they've waived their rights, actually have a copy of that LOR! - They have faith that the writer would never agree to recommend you if they truly thought you're a danger to patients or society, or they didn't believe you have the skills to be a physician

Great question. It depends on what you have been doing, and what you plan to do from now till residency to not lose touch with patients. Some continue to practice as a general practitioner in their home country, others come to the US to gain clinical experience, secure letters of recommendation and prepare for the USMLEs. Also keep in mind that in 2020 there will most likely be major changes to the USMLE, including the very real possibility of the test moving towards Pass/Fail - which may change the way you prepare for exams. As for research or publications: they are ranked very very low by program directors as factors to consider when offering residency interviews. Take a look at mk0nrmp3oyqui6wqfm.kinstacdn.com/wp-content/uploads/2018/07/NRMP-2018-Program-Director-Survey-for-WWW.pdf, page 45 (Internal Medicine) and you see that research is rated at 26% and 3.3/5 in importance, as compared to MSPE (85%, 4.3/5) and Step 1 score (95%, 4.2/5). So if applying to IM, then don't worry about research or publications. The story is way different if you're applying to much more competitive specialties (eg neurosurgery or dermatology).
