It's a 125B enclosure requiring 9v at less than 100mA.

Almost perfect, with two Mr.Monk-traps built in :D ...I would recommend getting rid of the metal flakes produced by drilling before closing the box ... and the left over nut was really teasing me to recheck the whole video at what time it appeared on the desk :D

Is this close to the JHS mod? I have a packrat, but only use the mod setting. The tone is perfect, but a pedal with top jacks would save space on my pedal board.

Not really. In fact this basically turns a regular phase 90 into a script version.

The LFO is analog. No trim pot for LFO bias or symmetry.
