Not only that- This is just act 1. By Act 3 you can get to over 30 AC. And you can do it on 2 characters- first one with Armour of Agility- is a special half-plate+2 for 17 AC, that lets you add your entire dex modifier, the best you can get in dex is 22, or +6, add to that a Viconia's Walking Fortress (shield) for +3, the shield itself lets you cast a warding bond so that's another +1 until long rest- the ability recharges on every long rest, Balduran's helmet gives another +1, and shield of faith spell caps it off to a permanent 30 AC for a first character, and half of all damage taken by them is transferred to someone else- effectively getting resistance to all damage types, also the Armour of Agility, Balduran's Helmet and Warding bond give a permanent +4 to all saving throws, and the Helmet makes it so that critical hits don't deal extra damage (a crit still guarantees a hit, just no longer doubles the damage- there are actually several items that give that perk- the guy in the video is wearing one such item- the shield). The next one can get Hellfire Armour- a Full plate+3, for 21 AC with no dex scaling cause heavy armour, that provides the wearer with heavy armour proficiency- so even casters can wear it. Add to that a cloak of protection, ring of protection, Wondrous Gloves, Defender's Flail, and Evasive shoes for +1 AC each, a shield+1 (there are a few of those in act 3) for another +3 AC, and a shield of faith, and we get 30 AC. The Barbarian or Monk can get some insane AC too, there are a few items that provide bonus AC as long as you don't wear anything classified as "armour".

There are different versions to the Raphael glitch, but it always refers to killing Raphael in Act 1 to get exp and (if no challenge-run-rule is stopping you from that) his armour. The ways to do this that I know are: Avoid him until you enter the Mountain Pass, where a forced long rest will take place and he will show up in your camp. Then either: A) cast silence where he is standing, enter the silence bubble and kill him while silence lasts (it only lasts 100 turns/10 minutes) B) go into Wildshape and kill him. Or do a different version (which I assume is what they are doing here): C) have one party member approach him and "lock" him in the cutscene, while another one approaches and does some shenanigans to also kill Raphael. I'm not sure how this version of the glitch works and from Truant's perspective we can't see what Morgana is doing. Edit: okay so judging by the stream, that last version of the glitch doesn't seem to work anymore (though I'm pretty sure I saw a video showcasing it on YT) and will just end the cutscene early. In all the versions I listed, the circumstances prevent the attacker to get into a cutscene with Raphael (Silence prevents cutscenes alltogether as nobody can talk while silenced; wildshape also prevents most cutscenes), therefore never meeting the condition for Raphael to leave and giving you all the time you need to strike him down. Since he's a devil and won't really die outside of hell, he will respawn. (Though, the reason why killing him this early on doesn't break his later encounters may not be that lorewise he cannot die, but rather because the devs programmed him to be revived for the other encounters, since he is an important story NPC; I don't know)

Not possible permanently. With spells, possible by act 1. Warding bond, reaction-shield, and Mirror Image give a total of +15 AC that he's not using, and he already has 26 AC in this video.

Honestly going 3 warlock 9 paladin is a really good shout. lets you go all in on charisma with pact of the blade AND means your saving throw aura will have the max possible boost to your allies. also helps to have a charisma character in your party for talking! Hope this helps

Recommend 7 pally / 5 warlock. You get all the good auras in pally and get access to great level 3 warlock spells (specifically HoH and counterspell).

I was gunna recommend Warlock Pally but it seems like everyone else beat me to the punch! Both of these options are good level splits, but I prefer 3 Warlock/ 9 Pally on honour mode and 5 Warlock/ 7 Pally on any lower difficulty since that results in you getting 3 attacks
