So many people accidentally kill someone or even multiple people during car crashes mostly. But it can also occur if they, for example, don't follow rules set by professionals. A few years ago someone was too lazy (they stated so themself) to follow the safety rules in his apartment only a few weeks after moving in and the whole building caught fire and I think 2 or 3 people died. Wasn't on purpose at all, but it was still his fault unfortunately
