'To continue' also means 'to keep happening/existing'. It is also used as an 'intransitive verb' , for example:- The story continues on page twelve. His depression continued even after his psychiatric treatment. The rain may continue for several days. Thanks for watching.

In the last usage you said in the usage of headline first example fifty killed in airline crash in the second clause you said fifty passenger killed how we identify in the elliptical clause which people were killed

If I have understood your question correctly, "Fifty killed... " means fifty passengers on board the crashed aeroplane were killed. But in the headline, the word ' passengers' is left out but understood. Hope this helps.

It depends. If the person's prolonged suffering is still going on at present, 'has been suffering' in present perfect continuous tense is the answer. However, if his suffering of months ended before a definite time in the past, the answer should be 'had been suffering' in past perfect continuous tense. Hope this helps.
