Should the trajectory of the free hanging lower leg be as close to fore/aft as opposed to a “duck like” outward swing, about 15 to 20 degrees away from straight fore/aft? I also ended up with a supplemental femur, +5.00” but the TKR was 1” shorter than the not op side. I read that the medical team was very “ Dimensionally Attentive”. +/- inches is not attentive, it’s not close. Please provide the more appropriate outcomes on leg length and leg trajectory.

I went to Dr Brett himmellwright in PA. He told me there is nothing special about superpath,that there isn't any advantages in recovery. Btw ,Dr himmellwright brought the superpath technique to PA,so go figure?

SuperPATH seems to be the lateral approach but modified and improved? It's also reported that if the surgeon faces problems they'll revert to cutting muscles and tendons? Maybe even making another incision? So what's the point? Is it just a lucky dip? Does the surgeon know if you are a good candidate who is likely to not have issues while being operated on?

Just had the SuperPath done in January. No pain at all! Had a traditional knee replacement and it took forever to heal. This hip replacement was the best!

My surgeon used this knee for my TKA 4 months ago. I now have 0-140 ROM, and fairly pain free except when going down stairs. I'll often get a sharp pain when doing this. My main issue now, and it's a big one for me, is the amount of clicking in knocking I get through swing phase. I am not 100% sure the doctor got my knee aligned correctly, or if I might have a slightly defective knee unit. I have talked to quite a few people who have had TKR and none have reported this amount of non wt bearing movement. I knew to expect from, but not had this extent. I'm pretty disappointed to say the least.

Six years ago Dr. William Kirk did a hip replacement on my left hip and it was amazing. I walked that day and in 4 weeks I was as good as new. I live in Southern Illinois about 300 miles from Nashville and my trip home was good!! Oct. 28 2021 I fell and broke my right hip and my husband wanted to take me to Dr. Kirk but I was in so much pain and said I would never make it down there so I went to one of our hospital in our area and boy was that a mistake!!The doctor that did my surgery was from Nashville and knew Dr. Kirk but is did not do the superpath he did the arterial approach. and he said he did it differently but the results were the same!! Sorry to say its not been the same! When I woke up my leg was numb from my knee up and a year later it's still numb! Also he left my right leg shorter then my left so that's been a problem in my walking! If I could have it done over with Dr. Kirks I would go in a minute but I don't think my insurance would pay for it!!! So I'm stuck!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

For a Patient with moderate Varus Knee Deformity & Arthritis(Both Knees), would Kinematic Total Knee Replacement reduce or minimize the Varus Deformity? If it does not reduce the Varus Deformity, seems like the implant would wear unevenly over time. How should this issue be addressed? Can you direct me to studies or research findings that show advantages of Kinematic & Mechanical TKR for Treating Varus Knee Deformity & Arthritis Conditions?

The whole concept of kinematic alignment is that you don't change the patients constitutional alignment. The surgeon "resets" the knee, including the alignment, to what it was before the patient developed the arthritis. So let's say you had 4 degrees of varus without arthritis, and with cartilage wear the varus is now 10 degrees, you should be back to 4 degrees after a surgery that has been performed with the kinematic alignment philosophy. Within certain ranges of varus/valgus the benefits should outweigh the cons. Traditional mechanical alignment results in a straight hip knee angle, but this also causes complications like anterior knee pain and overall unnatural forces in the knee and especially the collateral ligaments, which might lead to discomfort, pain, (midflexion) instability etc. About 20% of patients never reach the "forgotten knee"-state, which is not a small amount. Surgeons that are experienced with kinematic alignment (most famous one would be Stephen Howell) report better outcomes, and those people also performed studies. A similar approach is the "functional alignment", that accounts not only for cartilage wear, but also for ligament tension, meaning that the tension of the ligaments more or less direct the cuts and the components are implanted in a way that the ligaments are optimally tensed throughout the whole range of motion after implantation. This can be achieved very precisely with robotic navigation, for example. But also the instruments you see in this video allow for a functional alignment strategy, only (as far as I know) the gap balancing is possible only in extension, not in flexion. But, in my opinion, it's still better than mechanical alignment. I'm convinced it'll be slowly but surely replaced with kinematic and functional alignment strategies. If your varus or valgus deformity is so severe that one must consider uneven wear or other mechanical complications, an option would be to do a correction osteotomy first. Other options, especially if the ligaments are worn out due to the malalignment, are (partially) constrained implants. As you see, it's a very broad topic and I can't fully explain it in a KZbin comment section...

Steve, WoW !! I had the “Superpath Total Hip Replacement” surgery as well. My experience [although good] was completely different than yours. First, I was not able to walk without my walker until 15 days later. Pain medication is still mandatory even though I’m just over 16 days post-op. I’ve been told everyone’s different.

this approach still has to cut the piriformis tendon. that leads to more scar tissue, more painful recovery and some level of posterior hip instability.
