While all DOs learn of this, only a very small proportion actually employ it into practice. A lot of it has to do with encouragement and reinforcement in our clinical training. Personally, I did an extra year fellowship in this to learn more proficiency, and be able to offer more for my patients.

I did this a few years ago. 3 injection series. 1st injection was an insane amount of pain for my elbow. Searing pain for 8.5 hours straight. After a few weeks, ultrasound showed significant healing before/after with tears disappearing. 2nd and 3rd injections were fairly minimal with pain, nothing even close to 1st injection. A few weeks after the 3rd injection, ultrasound shows zero tears remaining. I'm throwing fastballs again in my mid 40s, feels great. Just be prepared for injection 1, set the day aside, have ice and painkillers at the ready.

Thanks for the comments, and glad it helped, but not so glad that it hurt so bad! Technique of performing makes a difference, and I find that most of my patients feel some pressure but not pain with these injections. I would also say that I can typically get a soft tissue injury, such as golfer's elbow in this video, better with one, maybe two tops PRP procedures. Technique, dosage, rehab, aftercare, procedure prep, injection guidance, all of these are huge factors that can determine whether this will be a successful procedure or not. We have also moved away from icing it after the procedure, have patients use heat every 3-4 hours they are awake and able to the first 72 hours, gets the blood flowing and the growth factors mobilizing to the area!

It’s definitely a modality that has been gaining momentum in medicine, but still many doctors have not been trained in this. I actually teach other physicians how to perform this at a local and national level.

This would change the lives of so many. I am scheduled for an MSK ultrasound at university of Louisville hospital- the only facility in Kentucky that offers it after 1.6 years of MRI and CT scans to see my ligaments and tendons

Fortunately for me it was one and done! Sometimes it takes two, I have never had a patient need more than that. If there is arthritis then that changes the picture, and depends on if the treatment is moreso targeting the arthritis pain or the ACL instability. If the former, usually needs 2, sometimes 3 PRP treatments; if the latter, usually one and done, sometimes two and done.

@@thebestever333 The way PRP is performed definitely makes a huge difference! I customize each PRP to the individual treatment, ensure I create an adequate dosage of the platelets and growth factors, and ensure the way I perform the procedure is meticulous, and then appropriate aftercare and rehab is done correctly. All of these factors make a HUGE difference in whether these procedures are successful or not.

I always say "exercise is the best medicine, but just like any medicine you can underdose, overdose, and/or take the wrong dose!" So getting a correct diagnosis so you can properly rehabilitate it is step 1. It can last until your growth plate fuses down, but there are some treatments that can help. If you are dealing with this chronically like you describe, then time to get in with a sports med physician like myself.

Part of the issue with this injury is awareness and getting providers to recognize the injury so treatments can begin early. It took my PCP 4 1/2 years to acknowledge this is what happened to me. I’m still under going treatment for it. Hopefully soon insurance will start to cover the PRP injections for treatment.

Well hopefully this video gets shared and increases awareness of this condition. I doubt insurance will cover PRP anytime soon as it doesn’t make them money, and insurance companies don’t care about their customers, they only care about their bottom line.

That is a good question. PRP has been shown to be effective in treating cartilage injuries. That being said, does PRP fix and repair torn and worn cartilage? Well, that is still up for debate and sometimes we see that is the case, sometimes not. But we do know that PRP allows the joint to functionally repair itself, so it is not swelling, aching, and preventing us from doing activities involving it.

I popped my calf muscle 3 weeks ago and my foot and ankle are in pain. I’ve rested, iced/heat. Why is does this feel like an ankle/foot sprain ? Makes no sense to me ?

Could be more than one injury. Also very often you tend to change the way you walk after an injury, could cause you to put pressure on the ankle. Ultimately if you are having an issue, I recommend you see a sports medicine specialist like myself to get definitive care!

I recently ruptured my plantaris tendon playing softball. I heard a pop when I went from a stationary position on first base to run to second base upon the ball being hit. It has been 4 weeks am. I am finally starting to walk again. But like you have mentioned, I tend to walk a little differently to compensate for the injury. I have been doing some light exercises but carefully not to overdo it. Just wondering if there are any must do and don'ts when recovering from this injury. I also believe that there is some damage to the calf muscle as well. But the doc seems to think it is solely my tendon. Thanks in advance for any suggestions!

@@stephenstagg4814 Have you had any imaging? I routinely perform diagnostic musculoskeletal ultrasound to better characterize these injuries. Can be very helpful to guide the appropriate treatment/therapy. Many of these injuries respond very favorably to properly guided physical therapy, and I commonly employ treatments such as shockwave, OMT, and at times use PRP (platelet rich plasma) and/or PPP (platelet poor plasma) if it is mainly a muscular injury.

One nsaid shouldn’t be a deal breaker, worse if you’ve been on them constantly. Hopefully it works great for you!
You can use the flex bar for a lot of the rehab. Isometric holds are the best thing I've noticed to repair my wrist and elbow a pain and although I may not be a doctor. I am data driven and this is by far the best for my myriad of problems. Keep with it. Look up a couple videos too there's one guy that made a comprehensive video on how to use it and you can't go wrong with it.

Doing this you are increasing the ascending ROM of the diaphragm, but doing nothing for the descending ROM. After you free up space under the ribcage with your fingers as you showed you should do the opposite motion, press and pull their abdominal muscles and diaphragm down as they inhale into their belly. If they breathe into their belly while you hold your fingers under their ribs you will both feel the diaphragm roll past your fingertips, after it does press in and pull down while they take a big breath so it doesn't ascend to the stuck position. If they are very stuck they will likely feel and hear a large pocket of air release from under the ribs. Anyone reading can try this on themselves.

The "Osteopathic Salute" was something routinely taught when I was a med student (and OMM Fellow) at TCOM over 20 years ago. I am not sure if they are still teaching it there or anywhere else.

Different physicians may have varying recommendations after this procedure. We find that a brace is not necessary unless it makes you feel better mentally.
