Thank you so much for this information! In the video, form 285 is referenced for the Home Office Cost Statement, but it appears that’s actually CMS Form # 287 (not 285).

The horrible conditions and financialization of elder care is all part of the class war, which is total war.

What about the office refused my rights to see or speak to my doctor when now a pattern of causing me to be without my medication, she admitted on recording do I think she cares about my medication I need.. I’ve had sepsis, I’m a dv survivor, and refused privacy, refuse mental health treatment, she exposes my history humility my rights asking to pleaser have consideration of me being a dv survivor and she and her other works laugh at me. No one will provide me help and lies about contacting the doctor, she makes me go in with drawl now every month my medication is needed, the script for some reason will have different directions that will cause the doc to always have verbal confirmation ever since I told on her when she refused me to see my doctor and I told her it isn’t her determination to decide if I am sick and she forces me to speak on my history n private information which she throws in my face as discrimination and laughing at me… it’s horrible

I have unearned income called RSDI. The person next to me in this nursing home is keeping her money. 2 patients fill up each small bedroom here. She says they take nothing from her. She is 45 and I am 69. She refused to sign their papers and she is still here. I could find no papers that said I should be letting go of my RSDI unearned income. $60 is no where near enough for a person that needs shoes and incontinent pads and wound bandages and organic foods. Therefore, I say I am being discriminated against, harshly. I have decided to begin keeping all of the $918 RSDI unearned income I receive every month. This nursing home is denying me incontinent pads and wound care bandages and the service of putting these bandages on my legs, plus several of the employees are outright abusive to the residents, meaning their voices are not kind and they take hours to change someone out of a diaper full of poo. Not enough people are hired, especially on weekends, so some patients have been heard to yell for hours. Without raw organic foods, metabolic acidosis will literally take over my body with acids so that my kidneys totally give out. I have asked for organic foods, but have been rejected. The video Genetic Roulette says that pesticides and GMOs are making us all sick. The last hospital I was in, diagnosed me with acute kidney injury and metabolic acidosis. My legs are the receivers of all the toxins my kidneys are not able to filter out. I have read much that says diet is the cause of acids running through the blood. This nursing home serves a huge amount of sugary desserts and juices and carbohydrates like potatoes and macaroni and meats. All of these are acidic and have been blamed for dementia and alzeimer's and all other dis-eases. Dr. Berg and Dr. Lustig speak in youtube videos about sugar being poisons. Dr. Young is one of these that says all of us should be eating alkaline foods. He has written several books and so has Dr. Lustig. I have read that we are alkaline beings. Our bodies regulate us at 7.35 pH to 7.45 pH on a regular basis. When our bodies become acidic, they look for alkaline substances like our bones and the cartilage between the bones. I didn't find this out until recently. I have no cartilage between my leg and hip bones, so I am always in a wheelchair and I sleep in a chair next to the bed because this bed is so hard and hurts me. Being severely abused as a child led me to eating too many processed foods, so I am about 100 pounds over weight. After spending many years watching videos and reading books and articles, I found out that the federal reserve is a cartel right along with the IRS. Corporations have taken over the government. The video "The Corporation" says all corporations are psychopathic. I have heard that these processed food corporations made a deal several decades ago with the government to cause the food pyramid to reflect these meats and carbohydrates as excellent nutrition. All carbohydrates turn into sugar when not exercised off the body. In a nursing home where many people are not moving, these sugars are dangerous stuff, even poisons. Sugar is being called a poison. This nursing home is literally poisoning the patients. Then there are the candy and coke machines. There are many people working here that are way too big body wise. The patients have access to these machines. Plus, there are employees that tell the patients when it is time to go outside to smoke cigarettes. NONE of this should be called nursing. This is not a nursing home. It is a for profit corporation. Is there precedence that allows me to keep my unearned income? Phyllis

The reason they are in ltc facilities is because we closed mental institutions in the 70's. It also the reason for the huge homeless and drug addict problem. We need to reopen institutions and house all these folks appropriately.

Residents certainly have many rights. Unfortunately, my experience has been that the facility tends to ignore those rights, as if to say what are you going to do about it. Ombudsmen or DOH don't always get things resolved and even if they do, it certainly isn't very timely. There needs to be much more transparency in how nursing home complaints are dealt with. You never hear back from DOH to tell you what was done.

Thank you so much for this information. It is so valuable to me a daughter who is trying hard to help my mother with dementia. I am getting no help from the nursing home who is forcing her out of her home without the benefit of allowing her family to have any input. It is a frustrating situation. I have found some help and a lot of hope from the Ombudsman of Hudson Valley.

@LTCCC: PLEASE ADD BRAIN INJURY Re: slide selected excerpts from the federal guidelines... STAFF to RESIDENT abuse of any type - Diverse populations included Please add residents with BRAIN INJURY specifically as an example of the diverse resident population in nursing home care! This is especially important as I've experienced 30 years of issues were my mother's caretakers continually created or contributed to behavioral issues, blaming her and even accusing her of willfully and intentionally acting out inappropriately. Specific training is necessary to understand the difference with brain injury patients, patients who once were just as capable and cognitive as their caregivers. The methods of interacting with and treating persons with BI differ from mental illness/disorders, mental retardation, dementia, Alzheimer's, or intellectual disability. RECOMMENDATION- Just 1 of many resources- (ex: internet search terms "brain injury challenging behaviors") The Brain Injury Institute of America website has help including links to quick reference guides and training options available, including online training/informational videos that benefit all nursing home care providers and all staff - ADMINISTRATOR , SOCIAL WORKERS, finance office and especially the front line health care providers - nursing staff members. www.biausa.org/brain-injury
