Central masking appeared in the first trial. But why did not you play 5-dB HL stimulus for twice to confirm his masked BC threshold? I saw you only play the 5-dB HL stimulus for once.

Please, as a hearing instrument specialist in training, it benefits me to SEE what the Audiologist is doing and HEAR the patient. Not the other way around. Great video otherwise, and thank you for your time and energy.

Thank you for your comment! Some states have specific ways they like to see exam candidates perform procedures. However, there are many ways to appropriately brace. The video was edited in a way that cuts some of the procedure out, which might be causing the confusion. New earmold impression videos are on our list to create, including videos that show different methods of bracing. Hopefully those videos will be of greater use to you!

We are glad to hear that! To answer your question, no. SRT is not the same as WRS. SRT is speech recognition threshold. WRS is word recognition score.

SRT is a speech version of pure-tone audiometry, which makes the stimulus sound softer and softer to seek a threshold. WRS is a suprathreshold test, which maintains the stimulus level the same through the entire test. Also, the SRT test uses spondees and has a familiarization section, while the WRS test uses monosyllables without a familiarization section.

Hi Eugenia! Thank you for pointing that out. We do not initially teach our students to use the occlusion effect. That is a procedure taught after they grasp the fundamentals of masking and bone conduction testing. Masking with the occlusion effect is on my list of video ideas for the future!

Thank you for your comment. This masking rule can be found in the Pure Tone Audiometry and Masking textbook written by Maureen Valente. There are multiple ways to mask for bone conduction. This is the foundational rule we teach our students.

​@@LetsRopeUp Hello! I am a student currently in WA state. We follow the IHS textbook very closely. As of yet I have not seen any protocols to apply masking in the NTE when there isn't an air-bone-gap. Could you clarify why you use this method VS masking only the gap. If my memory serves correct IA is 0 dB with BC? Is that the reason for masking? Is this a local regulation or perhaps specific to your company? Thanks for the info!

@@alec7083 Hi Alec! Masking for both sides gives you accurate scores for the bone conduction thresholds for both ears. Whether you use the rule or you apply masking all the time, you're testing correctly. Many times we teach students to mask both sides to they get lots of practice with masking. "Ah yes, that's okay" *knods* --> that's not uncomfortably loud. The tester should at least see the face of the patient. Muscle contractions in the face give away whether the sound is uncomfortably loud. And don't use continuous sound, but rather individual 1-second tones.

Thank you for your interest and comment! We agree with you about the importance of monitoring the patient and do teach our students to monitor facial queues when assessing loudness discomfort levels. This video is meant to be a very basic example of the LDL test, not a detailed instructional guide. Utilizing pulsed tones is common practice for finding LDLs.

Mam I took a test online speech test it was. They told me I would be unable to perceive the place of articulation in sentences . Does it mean I may suffer from high frequency hearing loss.

Thank you for your comment! I would suggest scheduling an appointment for a hearing evaluation with a hearing healthcare provider near you. You can talk with your primary care physician for a referral, contact your health insurance company, or search the web. They can give you a clear look at your hearing health and provide counseling as needed!

This is not SDT, SDT is Speech Detection Threshold. Speech Detection Threshold asks the patient to respond when they are first able to detect speech. They do not have to repeat words or understand them, only that there is speech coming through. You can find more information on ASHA's website: www.asha.org/policy/gl1988-00008/.

Greetings! The otoblock should not be pushed to the eardrum. It is normally placed just past the second bend in the ear canal. That placement may change depending on what style of mold you are ordering and the anatomy of the patient's ear. Earmold impressions should only be performed by trained professionals.

SRT starts at PTA (pure tone average). I think they’ve changed the rules since this was taken. ?

Dan, thank you for the comment! You can use MCL +10 as your starting level for WRS, for sure! Just like SRT starting levels, there are multiple correct ways of finding that starting level. As long as you can reference your method and it works for you and your patients then you are on the right track!

Hello! Speech audiometry is a pretty complex topic. We have two courses decided to audiometry in our online program. You might have better luck learning about the topic by purchasing a textbook or enrolling in a course dedicated to the topic. What is the patient's 'MCL' that you refer to around twenty six seconds? Maximum Comfort Level? P.S. Did you notice that the Audiometer spelled 'stairway' incorrectly? ['stariway'] ;}

MCL is the patient's Most Comfortable Listening Level recorded by using cold running speech and increasing or decreasing the presentation level until the patient reports a level where they normally listen to the TV or radio.
