I think a suoer cool way if implementing sanity into NPCs would be that when you're insane villagers start attacking you in their "insane state". You kill them so you don't die... But if you ever get back to reality they'll still be dead

i think to redesign the swimming reducing your sanity, swimming should increase your sanity when you're in a small source of water such as a river or pool. but rain would decrease it, because it's unpleasant.

I think it would be cool if you could put a serum in a flesh spike so any mob that walked on that spike would be affected, but it would run out of serum after a few hits
