Case 1:This patient has got significant mass effect with midline shift. I agree that tumour is chemo + radio sensitive, but when you go for radiation the mass effect is going to increase significantly. Being a Neurosurgeon our biggest concern is mass effect, how to address that concern?

Case 2: If this patient is symptomatic from anyone of the lesion that grows , why not go for excision of that symptomatic lesion instead of going for srs? Even for palliation you need to address the one that is life threatening (causing mass effect).
