For some reason, in my mind I was set on endothelin 1 only being the answer if it were like PAH. Thanks for helping bridge this gap

There would be pulmonary hypertension here. The high volume through the lungs from the ASD results in that.

Me too. This required a bit of critical thinking. He's getting PCP again so he must not me on prophylactic precaution. So he is at risk of Toxo (also treated with TMP-SMX prophylaxis).
